摘要
目的探讨胸腔内注射尿激酶治疗结核性胸膜炎临床效果及作用机制。方法选取2017年4~12月我院收住的60例结核性胸膜炎患者为研究对象,按照随机数字表法将所选对象分为对照组和观察组,各30例;两组均给予抗结核治疗,并进行胸水引流,观察组进行胸腔内尿激酶注射,而对照组则注入等量的生理盐水。观察并比较两组患者胸水引流时间、引流量、胸膜厚度以及肺功能变化。结果观察组患者胸水引流量、引流时间明显优于对照组(P<0.05),观察组拔管后胸膜厚度明显比对照组薄(P<0.05)。观察组患者胸膜粘连发生率明显低于对照组,病情好转率明显高于对照组(P<0.05);治疗前两组肺功能等相关指标比较差异无统计学意义(P>0.05),治疗后两组肺功能等相关指标均明显改善,观察组明显优于对照组(P<0.05)。结论对结核性胸膜炎患者,进行胸腔内尿激酶注射,有利于胸水的快速吸收,对胸膜增厚、胸膜粘连等情况的发生可起到明显的降低作用,促进治疗效果。
Objective To explore the clinical curative effect and mechanism of intrathoracic injection of urokinase in treatment of tuberculous pleurisy. Methods 60 patients withtuberculous pleurisy who were admitted and treated in our hospital from April 2017 to December 2017 were selected as research objects.According to the random number table method,they were divided into the control group and the observation group,with 30 cases in each group. Both groups were treated with anti-tuberculous therapy and hydrothorax drainage.The observation group was given intrathoracic injection of urokinase while the control group was given injection of same amount of normal saline. The hydrothorax drainage time,drainage volume,pleural thickness and lung function changes were observed and compared between the two groups. Results The hydrothorax drainage volume and drainage time in the observation group were significantly better than those in the control group (P < 0.05).After extubation,the pleural thickness of the observation group was significantly thinner than the control group (P < 0.05).The incidence of pleural adhesions in the observation group was significantly lower than that in the control group,and the improvement rate was significantly higher than that in the control group (P < 0.05).There were no significant differences in lung function and other related indexes between the two groups before treatment (P > 0.05).After treatment,the lung function and other related indexes were significantly improved in the two groups,and the observation group was significantly better than the control group (P < 0.05). Conclusion For patients with tuberculous pleurisy,intrathoracic injection of urokinase is beneficial to the rapid absorption of hydrothorax.It can significantly reduce the incidence of pleural thickening and pleural adhesion and it can promote the curative effect.
