摘要
目的探讨盆底超声在女性压力性尿失禁诊断及分级中的价值.方法选取2018年1月至2019年5月期间于本院行盆底超声检查的100例女性作为研究对象,根据是否存在压力性尿失禁分为尿失禁组(n=50)和无尿失禁组(n=50),所有入选者均行盆底超声检查,对比两组膀胱颈移动度(BND)、尿道旋转角(URA)、Valsalva状态膀胱尿道后角(PVUA);对比不同分级压力性尿失禁患者的盆底超声表现.结果尿失禁组BND、URA及PVUA水平均较无尿失禁组高,差异有统计学意义(P<0.05);34例压力性尿失禁患者中Ⅰ级15例,Ⅱ级11例,Ⅲ级5例,Ⅳ级3例;Ⅰ~Ⅱ级与Ⅲ~Ⅳ级患者尿道口漏斗形成率对比,差异无统计学意义;Ⅰ~Ⅱ级压力性尿失禁患者BND、URA及PVUA水平较Ⅲ~Ⅳ级低,差异有统计学意义(P<0.05).结论盆底超声在女性压力性尿失禁诊断及分级中具有较高价值,值得临床推广应用.
Objective To investigate the value of pelvic floor ultrasound in the diagnosis and classification of female stress urinary incontinence. Methods From January 2018 to May 2019, 100 female patients underwent pelvic floor ultrasound examination in our hospital were divided into two groups according to the existence of stress urinary incontinence group (n=50) and no urinary incontinence group (n=50). The pelvic floor ultrasound was performed in all subjects, and the bladder neck movement of (BND), was compared with that of (URA), Valsalva posterior angle of bladder and urethra (PVUA);of posterior angle of bladder and urethra was compared between the two groups. Comparison of pelvic floor ultrasonography in patients with different grades of stress urinary incontinence Now. Results The levels of BND,URA and PVUA in the group of urinary incontinence were higher than those in the group without urinary incontinence (P<0.05), among the 34 cases of stress urinary incontinence, 15 cases were grade Ⅰ, 11 cases were grade Ⅱ, 5 cases were grade Ⅲ, and 3 cases were grade Ⅳ. There was no significant difference in the formative rate of urinary canal funnel between grade Ⅰ-Ⅱ and grade Ⅲ-Ⅳ. The levels of BND, URA and PVUA in patients with grade Ⅰ-Ⅱ stress urinary incontinence were lower than those in grade Ⅲ-Ⅳ(P<0.05). Conclusion Pelvic floor ultrasound has high value in the diagnosis and grading of female stress urinary incontinence. It is worth popularizing and applying in clinic.
