面神经MR成像技术及临床应用

摘要 目的探讨磁共振成像技术在面神经功能障碍患者中的应用诊断价值。方法选取2017年6月-2017年10月间来我院进行治疗的单侧面肌痉挛患者60例以及非面肌痉挛患者60例分别作为研究组及对照组,采用MR成像技术观察患者患侧及健侧面神经脑池段受其周围血管压迫导致的原发性面肌痉挛形态学特征。结果对照组患者面神经脑池段与其周围血管相交处的血管直径为(0.48±0.02)mm,研究组健侧(0.49±0.03)mm、患侧(0.72±0.05)mm,随着血管直径的增加,面肌痉挛患者的发病率逐渐增加;对照组患者经血管相交处与脑干的距离为(8.54±0.62)mm,研究组健侧(8.64±0.57)mm、患侧为(5.86±0.41)mm,随着距离的增加,面肌痉挛患者的发病率也逐渐降低;对照组患者面神经周围血管的自身角度(74.28±3.10)°,研究组健侧(74.36±2.97)°、患侧(47.92±3.21)°,随着自身角度的增加,面肌痉挛患者的发病率逐渐降低。结论面神经MR成像技术显示的面神经与其周围血管相交处的血管形态学评价指标对诊断面肌痉挛有重要的参考价值,值得临床推广使用。 Objective To investigate the diagnostic value of magnetic resonance imaging in patients with facial nerve dysfunction. Methods Sixty patients with unilateral tendon and 60 patients with non-facial spasm who were treated in our hospital from June 2017 to October 2017 were enrolled as the study group and the control group. MR imaging technique was used to observe the affected side of the patient. The morphological features of the primary hemifacial spasm caused by compression of the surrounding cerebral cistern. Results The diameter of the vessel at the intersection of the facial cerebral cistern and its surrounding vessels in the control group was (0.48±0.02) mm. The healthy side of the study group (0.49±0.03) mm and the affected side (0.72±0.05) mm, with the increase of vessel diameter. The incidence of hemifacial spasm patients gradually increased;the distance between the vascular intersection and the brainstem in the control group was (8.54±0.62) mm, the healthy side of the study group (8.64±0.57) mm, and the affected side (5.86±0.41) mm. With the increase of distance, the incidence of hemifacial spasm patients gradually decreased;in the control group, the angle of the blood vessels around the facial nerve was (74.28±3.10)°, the healthy side of the study group (74.36±2.97)°, the affected side (47.92±3.21)°, with the increase of their own angle, the incidence of hemifacial spasm patients gradually decreased. Conclusion The morphological evaluation index of the facial nerve at the intersection of the facial nerve and its peripheral blood vessels has important reference value for the diagnosis of facial spasm, which is worthy of clinical application.
