白细胞、生长停滞特异性基因产物6、血清淀粉样蛋白A联合在细菌性肺炎与病毒性肺炎临床诊断中的研究 被引量:6

Clinical value of WBC,GAS6 and SAA in the diagnosis of bacterial pneumonia and viral pneumonia

摘要 目的探究白细胞(WBC)、生长停滞特异性基因产物6(GAS6)、血清淀粉样蛋白A(SAA)联合在细菌性、病毒性肺炎中的诊断价值。方法细菌性肺炎患儿、病毒性肺炎患儿和健康儿各50例,检测外周血WBC以及血清GAS6、SAA水平,分析其单独检测和联合检测对肺炎的诊断效能。结果相比于对照组,细菌组患儿的WBC和SAA水平显著升高,而病毒组WBC和SAA水平显著降低(P <0. 05)。病毒组和细菌组的GAS6均显著高于对照组,并且细菌组的GAS6显著高于病毒组(P <0. 05)。WBC、GAS6、SAA平行联合检测的敏感度和阴性预测值均显著高于单独检测(P <0. 05)。系列联合检测的特异度和阳性预测值均显著高于单独检测和平行联合检测(P <0. 05)。结论对于细菌性肺炎和病毒性肺炎的鉴别诊断,WBC、GAS6、SAA平行联合检测可提高敏感性和阴性预测值,而系列联合检测可提高特异度和阳性预测值。 Objective To explore the value of white blood cell( WBC),growth arrest specific gene product 6( GAS6),serum amyloid A( SAA) in the combined diagnosis of bacterial and viral pneumonia. Methods 50 children with bacterial lung,50 children with viral pneumonia and 50 healthy children were conducted for the detection of peripheral blood WBC and serum GAS6,SAA levels. The diagnostic efficacy of separate detection and combined detection for pneumonia was analyzed.Results WBC and SAA levels in the bacterial group were significantly higher than that in the control group. The WBC and SAA levels in the virus group significantly decreased( P < 0. 05). GAS6 in the virus group and the bacterial group were significantly higher than that in the control group,and the GAS6 level of the bacterial group was significantly higher than that of the virus group( P < 0. 05). The sensitivity and negative predictive value of parallel combined detection of WBC,GAS6 and SAA were significantly higher than those of the single test( P < 0. 05). The specificity and positive predictive value of the serial combination test were significantly higher than the single detection and parallel combined detection( P < 0. 05). Conclusion For the differential diagnosis of bacterial pneumonia and viral pneumonia,parallel combined detection of WBC,GAS6,and SAA can increase sensitivity and negative predictive value,while serial combined detection can increase specificity and positive predictive value.
