摘要
目的探究居家腹膜透析患者有效实用的自我管理方法。方法采用扎根理论研究法对11例居家腹膜透析患者进行深入访谈,整理并分析访谈资料,找出代码、形成类属及核心类属。结果居家腹膜透析患者自我管理的核心类属为腹膜透析内化处理,呈现出4种不同的内化处理特征,即:融入生活的自由腹膜透析、按照处方进行的机械腹膜透析、调节失衡的被动腹膜透析和出现危机后的将就腹膜透析。结论医护人员应根据腹膜透析患者的不同内化处理类型提供针对性自我管理指导,使之既能节省护士人力资源、减轻临床中不必要的工作量,又能更有效地提高腹膜透析患者的自我管理水平。
Objective To explore more effective guidelines for self-management of peritoneal dialysis patients at home.Methods The grounded theory method was used to conduct in-depth interviews with 11 home-based peritoneal dialysis patients.The interview data were collated and analyzed,and the codes,genera and core genera were identified.Results The core category of self-management of home-based peritoneal dialysis patients was peritoneal dialysis internalization,which presented four different internalization characteristics:free peritoneal dialysis integrated into life,mechanical peritoneal dialysis according to prescription,passive peritoneal dialysis adjusted for imbalance and peritoneal dialysis after crisis.Conclusions It is suggested that medical staff should provide corresponding self-management guidance for the peritoneal dialysis patients under the different internalized treatment,it can not only save nurses’human resources and the unnecessary clinical workload,but also improve the self-management level of peritoneal dialysis patients more effectively.
