摘要 目的探讨高频超声联合彩色多普勒技术在腋窝淋巴结检查中的应用价值。方法选取陕西省肿瘤医院肿瘤体检中心2018年1月至2019年1月参加常规体检的20033例受检人员作为研究对象,采用高频超声联合彩色多普勒技术对腋窝淋巴结进行检查,记录可疑淋巴结的影像及诊断结果。结果检出腋窝淋巴结异常者103例,阳性率0.51%,恶性病变42例,其中转移灶37例,淋巴瘤5例;良性病灶共61例,其中反应性增生54例(急、慢性炎性22例,猫抓病2例,余例无明显病因),结核性病灶7例。高频超声淋巴结分类结果为,Ⅰ类38例(良性30例,恶性8例),Ⅱ类32例(良性19例,恶性13例),Ⅲ类24例(良性7例,恶性17例)、Ⅳ类9例(良性5例,恶性4例)。结论高频超声联合彩色多普勒技术能够通过腋窝淋巴结的形态结构及血流情况对其性质进行初步判定,指导临床早期进行穿刺活检等干预,在常规体检方面具有很大的临床实用价值。 Objective To explore the application value of high frequency ultrasound combined with color Doppler technology in axillary lymph node examination.Methods A total of 20 033 patients participated in routine physical examination from January 2018 to January 2019 at the tumour physical examination center of Shaanxi Provincial Cancer Hospital were selected as the research objects.The axillary lymph nodes were examined by high frequency ultrasound combined with color Doppler technology,and the images and diagnostic results of suspected lymph nodes were recorded.Results A total of 103 cases of axillary lymph node abnormalities were detected,the positive rate was 0.51%.There were 42 cases of malignant lesions,including 37 cases of metastasis and 5 cases of lymphoma.There were 61 cases of benign lesions,including 54 cases of reactive hyperplasia(22 cases of acute and chronic inflammation,2 cases of cat scratch disease,and the remaining cases had no obvious etiology)and 7 cases of tuberculosis lesions.The results of high-frequency ultrasound classification of lymph nodes showed that there were 38 cases of classⅠ(30 cases of benign lesions and 8 cases of malignant lesions),32 cases of classⅡ(19 cases of benign lesions and 13 cases of malignant lesions),24 cases of classⅢ(7 cases of benign lesions and 17 cases of malignant lesions)and 9 cases of classⅣ(5 cases of benign lesions and 4 cases of malignant lesions).Conclusion High frequency ultrasound combined with color Doppler technology can preliminarily determine the nature of axillary lymph nodes by their morphological structure and blood flow,and guide early clinical interventions such as puncture biopsy,which has great clinical practical value in routine physical examination.
