STAT3和HIF-1α在子宫内膜腺癌中的表达及其临床意义

摘要 目的探讨信号传导及转录激活因子3(STAT3)和缺氧诱导因子1α(HIF-1α)在子宫内膜的增殖期、不典型增生及腺癌中的表达,分析其与临床预后的关系。方法收集河南科技大学第一附属医院2011年1月至2013年12月刮宫或者手术切除的存档石蜡标本,根据子宫内膜组织类型分为3组:增殖期组、不典型增生组和子宫内膜腺癌组,统计子宫内膜腺癌的临床分期、病理分级、淋巴结是否转移及年龄等,应用免疫组化技术比较STAT3和HIF-1α在3组中的表达情况(SP法),分析其与子宫内膜腺癌及其5a生存率的关系。结果STAT3和HIF-1α在3组中的阳性表达差异具有统计学意义,子宫内膜腺癌组的STAT3和HIF-1α表达较其他2组增高(P<0.05),其中STAT3和HIF-1α表达与子宫内膜腺癌的临床分期、组织分级及淋巴结转移相关(P<0.05)。而且,HIF-1α的异常表达和STAT3的阳性表达呈正相关,二者与子宫内膜腺癌的5a生存率差异有统计学意义。结论STAT3和HIF-1α在子宫内膜腺癌的恶性转化过程中可能起协同作用,联合检测STAT3和HIF-1α对于判断子宫内膜腺癌的发生、发展及预后有一定价值。 Objective To investigate the expression of signal transducers and activators of transcription 3(STAT3)and hypoxia-inducible factor 1α(HIF-1α)in tissue of endometrium proliferative phase,endometrial atypical proliferation and human endometrial adenocarcinoma,and analyze its relationship with the clinical prognosis.Methods The archived paraffin specimens removed by curettage or surgerywere collected from the First Affiliated Hospital of Henan University of Science and Technology from January 2011 to December 2013.According to the pathological tissue types all samples are divided into groups of endometrium proliferative phase,endometrial atypical proliferation and human endometrial adenocarcinoma.The clinical stage,pathological grade,lymph node metastasis and age of endometrial adenocarcinoma were analyzed.The STAT3 and HIF-1αprotein were detected by immunohistochemical technique(SP method).The relationships were analyzed between STAT3 and HIF-1αprotein and endometrial adenocaricinoma and 5a survival rate.Results There were statistically significant differences of positive expression of STAT3 and HIF-1αin the three groups.The expression of STAT3 and hif-1αin the endometrial adenocarcinoma group was higher than those in the other two groups(P<0.05).The expression of STAT3 and HIF-1αwas correlated with the clinical stage,histological grading and lymph node metastasis of endometraial adenocarcinoma(P<0.05).Furthermore,the abnormal expression of HIF-1αwas positively correlated with the positive expression of STAT3,and there was a statistically significant difference between them and the 5-a survival rate of endometrial adenocarcinoma.Conclusion STAT3 and HIF-1α-may play a synergistic role in malignant transformation of endometrial adenocarcinoma.The combined detection of STAT3 and HIF-1αwould have certain value in determining the occurrence,development and prognosis of endometrial adenocarcinoma.
