摘要
目的观察联合应用可注射富血小板纤维蛋白(injectable platelet-rich fibrin,i-PRF)和自体脂肪移植矫正上睑凹陷的临床应用效果。方法收集2017年6月至2018年11月于我院整形美容科门诊就诊的32例上睑凹陷患者,根据患者上睑皮肤松弛情况分为经皮肤及经重睑切口两种手术入路方式,将分离提取的脂肪颗粒注射填充于上睑凹陷处眼轮匝肌后脂肪(ROOF)层,同时将离心得到的i-PRF均匀注射于上睑皮肤老化部位的真皮深层和皮下层。结果术后1例患者矫正不足,行2次手术后获得满意效果。1例患者不满意,感觉上睑组织堆积显臃肿。30例患者在1次注射填充后即达到满意效果,上睑凹陷基本矫正,上睑皮肤老化症状较术前很大改善,无一例出现感染、脂肪液化、脂肪坏死及脂肪栓塞等并发症。患者均获随访,随访时间3~12个月,平均6个月。结论联合i-PRF及自体脂肪颗粒移植可以有效地填充上睑凹陷,并且同时改善上睑皮肤老化症状,达到年轻化的满意效果。
Objective To investigate the clinical effect of a combination of injectable platelet-rich fibrin(i-PRF)and autologous fat transplantation for the correction of sunken upper eyelid. Methods For skin relaxation of the upper eyelid,transcutaneous and trans-incisional approaches were used in 32 outpatients who presented to the department of plastic surgery from June 2017 to November 2018. The separated and extracted fat granules were injected into the retroorbicularis oculi fat(ROOF)layer at the upper eyelid depression,and the i-PRF obtained by centrifugation was uniformly injected into the deep and subcutaneous layers of the dermis of the upper eyelid skin.Results One patient had insufficient correction after operation,and satisfactory results were obtained after two operations. One patient was dissatisfied with the accumulation of upper eyelid tissues and felt bloated. However,the other 30 patients achieved satisfactory results after one injection. The depression of upper eyelid was basically corrected,and the aging symptoms of upper eyelid skin were greatly improved compared with that before surgery.There were no complications such as infection,fat liquefaction,fat necrosis and fat embolism. All patients were followed up for 3 to 12 months(mean 6 months). Conclusions Combined with i-PRF injection and autologous fat granule transplantation can effectively fill the depression of upper eyelid. Meanwhile,the aging symptoms of upper eyelid skin can be improved to achieve a satisfactory effect of rejuvenation.
