摘要
目的分析痛风患者的生存质量及其影响因素。方法选择206例痛风患者为研究对象,收集所有患者的临床资料。采用世界卫生组织生存质量测定量表简表、抑郁自评量表、家庭功能评估量表和社会支持评定量表对所有患者进行调查。比较不同抑郁程度、家庭功能状况、社会支持水平患者生存质量各领域得分。采用Logistic回归模型分析痛风患者生存质量的影响因素。结果痛风患者生存质量生理领域、心理领域、社会关系领域和环境领域的得分分别为(44.85±12.16)分、(43.82±9.86)分、(48.31±11.23)分、(51.09±11.26)分,生存质量的自我评分为35~97(78.78±12.65)分。中、重度抑郁者生存质量各领域得分均低于无抑郁者;重度家庭功能障碍者的生理领域、心理领域及社会关系领域得分均低于无障碍者及中度障碍者;高水平社会支持痛风患者的生理领域、心理领域及环境领域得分均高于中、低水平社会支持痛风患者(均P<0.05)。饮食控制、月收入、痛风家族史、家庭功能、社会支持、饮酒史、入院次数、高血压病史和抑郁状况均是痛风患者生存质量的影响因素(均P<0.05)。结论痛风患者生存质量较差,其生存质量受家庭功能状况和社会支持水平等多种因素影响。
Objective To analyze quality of life and its influencing factors in gout patients. Methods A total of 206 patients with gout were selected as subjects,and the clinical data of all patients were collected.All patients were surveyed using the World Health Organization Quality of Life Scale-Brief Version,the Self-Rating Depression Scale,the Family Function Assessment Scale,and the Social Support Rating Scale.The scores for each domain of quality of life were compared among patients with different degrees of depression,different family function status,or different levels of social support.The influencing factors of quality of life in patients with gout were analyzed using Logistic regression model. Results The scores for physiological,psychological,social and environmental domains of quality of life in gout patients were 44.85±12.16 points,43.82±9.86 points,48.31±11.23 points and 51.09±11.26 points,respectively, the self-rating score for quality of life was 78.78±12.65(range,35-97) points.Patients with moderate or severe depression had lower scores for all domains of quality of life as compared with individuals without depression;individuals with severe family dysfunction exhibited lower scores for physiological,psychological and social domains than those without family dysfunction or with moderate family dysfunction;gout patients with high-level social support reported higher scores for physiological,psychological and environmental domains than those with moderate or low-level social support(all P <0.05).Diet control,monthly income,family history of gout,family function,social support,drinking history,admission times,history of hypertension,and depression state were influencing factors of quality of life in gout patients(all P <0.05). Conclusion Gout patients experience poor quality of life,whose quality of life is affected by various factors such as family function status and social support level.
