摘要
目的分析糖尿病患者实施医院联合社区协调共管模式的干预效果。方法选取2016年7月-2017年7月在福田区益田社区健康服务中心实施医院-社区一体化管理的196例糖尿病专案患者,随机分为对照组和观察组,每组各98例。对照组患者接受常规护理,观察组患者接受医院联合社区协调共管模式护理干预。观察两组患者生存质量评分、血糖(空腹血糖、餐后2 h血糖和糖化血红蛋白)水平、血脂(总胆固醇、三酰甘油、高密度脂蛋白胆固醇和低密度脂蛋白胆固醇)水平并进行比较分析。结果观察组各项生存质量指标的评分均高于对照组(P<0.05),观察组空腹血糖、餐后2 h血糖和糖化血红蛋白水平均低于对照组(P<0.05),观察组总胆固醇、三酰甘油和低密度脂蛋白胆固醇水平均低于对照组(P<0.05)。结论糖尿病患者接受医院联合社区协调共管模式护理干预可以有效提高患者的生存质量,改善血糖水平,降低血脂水平,该方法具有较高的临床价值,值得推广应用。
OBJECTIVE To analyze the intervention effect of coordinated community management model in the hospital of diabetes patients.METHODS 196 patients with diabetes who were nursing in target hospital from July 2016 to July 2017 were randomly divided into control group and observation group,with 98 cases in each group. The patients in the control group received routine care,and the observation group received the hospital joint community coordination model nursing intervention. The patients quality of life score,blood sugar levels(fasting plasma glucose,2 h postprandial blood glucose,glycosylated hemoglobin),lipid levels(three acyl glycerin,total cholesterol,high-density lipoprotein cholesterol and low density lipoprotein cholesterol)of these two groups were observed and compared. RESULTS The score of quality of life index observation group were higher than the control group(P<0.05). Fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin levels of the observation group were lower than the control group(P<0.05). The acyl glycerin,total cholesterol,low-density lipoprotein cholesterol levels of the observation group were lower than the control group(P<0.05). CONCLUSION Patients with diabetes hospital joint coordinate community condominium nursing intervention can effectively improve the quality of survival,improve blood sugar,reduce blood fat levels. The method has high clinical value,which is worth popularizing application.
