摘要
目的研究改良皮肤护理模式对肛瘘术后患者生活质量及预后的影响,为患者的术后护理提供指导。方法选取2017年6月-2018年6月在上海中医药大学附属曙光医院接受诊治的120例肛瘘患者患者。按照随机数表法,分为对照组和观察组,各60例。对照组患者给予传统护理,观察组给予改良皮肤护理模式护理。比较两组患者术后4 d、14 d和24 d的瘙痒情况,术后24 d的湿疹发生率、湿疹面积及痊愈时间,护理干预前后的生活质量和并发症发生率。结果术后4 d、14 d和24 d,观察组患者的瘙痒率均显著低于对照组,差异有统计学意义(P<0.05);术后24 d观察组患者中湿疹发生率显著低于对照组,且湿疹面积较小,痊愈时间较短(P<0.05);护理后,观察组生活质量评分明显高于对照组(P<0.05);观察组患者并发症发生率明显低于对照组。结论改良皮肤护理模式对肛瘘术后患者进行护理,有助于缓解患者的肛周皮肤湿疹症状,保护肛周皮肤,提高患者的术后生活质量,并减少并发症的发生率,有助于预后,具有临床推广价值。
OBJECTIVE To study the effect of skin care model on the quality of life and prognosis of patients after anal fistula,and provide guidance for patients’ postoperative care. METHODS A total of 120 cases anal fistula patients who were treated in target hospital from June 2017 to June 2018 were enrolled in the study and divided into the observation group and the control group according to the random number table method(n=60). The control group were treated with a traditional care model,and the observation group were treated with a modified skin care model. The pruritus at 4 d,14 d and 24 d after operation,the incidence of eczema,the area of eczema and recovery time after 24 days,the quality of life and the incidence of complications before and after nursing intervention were compared. RESULTS At 4 d,14 d and 24 d after operation,the pruritus rate of the observation group(8.33%;1.67%;0.00%)was significantly lower than the control group(38.33%;38.33%;36.67%),the difference was statistically significant(P<0.05). On the 24 th day after operation,the incidence of eczema in the observation group was significantly lower than the control group,and the area of eczema was small,and the recovery time were shorter,the difference was statistically significant(P<0.05). After nursing,the quality of life scores of the observation group were significantly higher than the control group;the incidence of complications in the observation group(3.33%)was significantly lower than the control group(13.33%,P< 0.05). CONCLUSION Using the modified skin care model to care for patients after anal fistula could help relieve and treat the symptoms of perianal skin eczema,protect the perianal skin,improve the quality of postoperative life,reduce the incidence of complications and help prognosis,which had clinical promotion value.
