摘要
放射性肺损伤是胸部恶性肿瘤放射治疗的主要不良反应之一,严重影响患者生活质量。中医治疗放射性肺损伤疗效确切,不同医家对放射性肺炎的辨证、治法、方药各不相同。本文主要从中医对放射性肺炎的病机、辨证、治疗几个方面加以综述。放射性肺炎基本病机为热毒耗伤气阴,损伤血络,热毒蕴结。辨证以肺燥阴(津)亏、热毒犯肺、痰瘀互结为主,且以阴虚证型为主。经方与验方在放射性肺炎的治疗中都显现出很好的疗效,通过各家选方用药可以看出放射性肺炎的治法以养阴生津为主,兼以清热、活血,其疗效也通过基础与临床试验得以验证。
Radiation-induced lung injury (RILI) is one of the main side effects of radiotherapy for thoracic malignant tumor, which seriously affects the quality of life of patients. The efficacy of traditional Chinese medicine (TCM) on RILI is clear. Different doctors’ viewpoints of the syndrome differentiation, treatment and prescription in RILI are different. This paper summarizes the pathogenesis, syndrome differentiation and treatment of RILI in TCM. The basic pathogenesis of RILI is that heat toxin consuming qi and yin , damaging blood collaterals, and heat toxin accumulation. Syndrome differentiation includes lung dryness and yin deficiency, heat toxin invading the lung, and binding of phlegm and stasis, and yin deficiency is the main syndrome. Classical prescriptions and empirical prescriptions in the treatment of RILI have shown very good efficacy. From various prescriptions of doctors, it can be seen that the main therapeutic method includes nourishing yin and promoting fluid production mainly as well as clearing heat and invigorating blood, and its efficacy has been verified through the basic and clinical trials.
