“表皮、黑素单元”抑或“表皮、真皮、黑素单元”?——对皮肤色斑形成机制的思考 被引量:4

摘要 本文综述了皮肤色斑的临床现象及近年的基础研究成果,跳出现有皮肤色斑形成机制的局限性,以皮肤表皮和真皮作为一个整体,从系统论的角度以更广的范围和思路看待皮肤色斑的形成和调节,初次提出了"表皮、真皮、黑素单元"构想,并为此概括了两种色斑调节机制,包括A模式--初始点状色斑:启动调节细胞可能为淋巴细胞等;B模式--初始为斑片:启动调节细胞可能为血管细胞或者血管本身.这两种模式可以同时或者先后启动,比如先是细胞启动调节形成斑点,然后血管启动调节扩大为斑片,也可以在表皮的不同部位分别启动A模式和B模式,形成斑点或者斑片.表现出临床色斑的多形性. The first,this article reviewed the history of pigmented spot and patch .The second,the arthor proposed a new mechanism of pigmented spot and patch adjust as Epidermal-dermal melanin unit for the first time.The hypothesis concerned the key role of dermal adjusting melanin formula in epidermis.Cells in epidermis were considered as the effector of the other cells in dermal .At least,two models involved in the hypothesis.Model A,pigmented spot model ,spot was started and adjusted by dermal cells primarily such as lymphcytes.Model B,pigmented patch model,patch may be acted and adjusted by dermal vascular.
