肺癌患者DCE-MRI定量灌注直方图参数与肿瘤组织微血管密度的相关性 被引量:10

Correlation between quantitative perfusion histogram parameters of Dynamic Contrast-Enhanced MRI and tumor tissue microvessel density in patients with lung cancer

摘要 目的探讨肺癌患者动态对比增强磁共振成像(DCE-MRI)定量灌注直方图参数与肿瘤组织微血管密度(MVD)的相关性。方法收集2017年3月至2018年3月在我院就诊,术前进行DCE-MRI扫描且病理证实为肺癌的30例患者。Exchange模型测量各定量灌注参数血管内至血管外细胞外间隙转运系数(Ktrans)、血管容积积分(Vp)、血管外细胞外间隙返回至血管内速率常数(Kep)、血管外细胞外间隙容积积分(Ve)、血浆灌流量(Fp)的直方图参数(中位数、平均值、偏度、峰度、能量、熵、百分位数)。免疫组化方法检测肿瘤组织中CD34的表达,显微镜下计数微血管数。采用SPSS 19.0软件进行统计学分析,用Pearson直线相关分析评价MVD值与exchange模型测量的各DCE-MRI定量灌注直方图参数的相关性。结果3组不同病理类型肺癌中MVD值差异无统计学意义(P>0.05);Exchange模型中各定量灌注直方图参数组内分析差异无统计学意义(P>0.05)。Exchange模型中Ktrans(均数、25%位数、50%位数、75%位数、90%位数、95%位数)、Kep(熵、10%位数、25%位数、50%位数、75%位数、90%位数)、Fp(均数、25%位数、50%位数、75%位数、90%位数、95%位数)、Vp(熵、75%位数、90%位数、95%位数)与MVD呈正相关(P均<0.05);Ktrans(能量)、Vp(偏度、峰度、能量)与MVD呈负相关(P<0.05)。Ve灌注直方图参数与MVD无显著相关性(P>0.05)。结论DCE-MRI定量灌注直方图参数与MVD存在相关性,可以用于间接评价肺癌组织的MVD情况。 Objective To investigate the correlation between quantitative perfusion histogram parameters of DCE-MRI and tumor tissue microvessel density(MVD) in patients with lung cancer. Methods 30 patients with lung cancer confirmed by pathology who underwent preoperative DCE-MRI were enrolled in this retrospective study. Quantitative perfusion histogram parameters(including median, mean, skewness, kurtosis, energy, entropy) were measured for each patient using Exchange modle. Using the Immunohistochemical method to detect the expression of CD34 in tumor tissue, and counting the number of microvessels under microscope. SPSS 19.0 was used to carry out statistical analysis. The correlation between MVD and quantitative perfusion histogram parameters of DCE-MRI measured by exchange model was evaluated by Pearson linear correlation analysis. Results There was no significant difference in MVD and each quantitative perfusion histogram parameters between the three different pathological groups of lung cancer(P>0.05). Ktrans perfusion histogram parameters(mean, 25%, 50%, 50%, 75%, 90%, 95%), Kep perfusion histogram parameters(entropy, 10%, 25%, 50%, 75%, 90%), Fp perfusion histogram parameters(mean, 25%, 50%, 75%, 90%, 95%) and Vp perfusion histograms parameters(entropy, 75%, 90%, 95%) were positively correlated with MVD(P<0.05). Ktrans perfusion histogram parameters(energy) and Vp perfusion histogram parameters(skewness, kurtosis, energy) were negatively correlated with MVD(P<0.05). There was no significant correlation between Ve perfusion histogram parameters and MVD(P>0.05). Conclusion There was a certain correlation between the perfusion histogram parameters of DCE-MRI and MVD, suggesting that the quantitative perfusion histogram of DCE-MRI in lung cancer can reflect the MVD value of cancer tissue.
