人工股骨头置换术与防旋股骨髓内钉治疗股骨粗隆间骨折的效果及安全性 被引量:1

Effect and safety of artificial femoral head replacement and proximal femoral nail anti-rotation in the treatment of intertrochanteric fracture of femur

摘要 目的研究人工股骨头置换术与防旋股骨髓内钉(PFNA)治疗股骨粗隆间骨折的效果与安全性。方法选择我院2015年5月至2018年3月收治的75例股骨粗隆间骨折患者为研究对象,根据手术方式不同将其分为A、B组,分别为38、37例。其中A组患者均采用人工股骨头置换术进行治疗,B组患者均采用PFNA进行治疗。对比两组患者的治疗效果与安全性。结果B组患者手术切口长度、术中出血量与手术时间均优于A组,但术后开始站立与行走时间则长于A组(P<0.05);术后6、9个月,B组患者髋关节Harris评分高于A组(P<0.05);A组患者不良反应总发生率为13.2%,B组不良反应总发生率为5.4%,两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论人工股骨头置换术与PFNA治疗股骨粗隆间骨折均能达到良好的临床效果,且安全性高,可针对患者不同情况选择手术方式,有助于促进患者尽早康复。 Objective To research the effect and safety of artificial femoral head replacement and proximal femoral nail anti-rotation(PFNA)in the treatment of intertrochanteric fracture of femur.Methods A total of 75 cases of patients with intertrochanteric fracture of femur treated in our hospital from May 2015 to March 2018 were selected as the research objects and divided into group A(n=38)and group B(n=37)according to different surgical methods.The group A was treated with artificial femoral head replacement,while the group B was treated with PFNA.The treatment effects and safety were compared between the two groups.Results The incision length,intraoperative bleeding volume and operation time in the group B were better than those in the group A,but the time of standing and walking after operation were longer than those in the group A(P<0.05);the Harris scores of hip joint at postoperative 6 and 9 months in the group B were higher than those in the group A(P<0.05);the total incidence of adverse reactions in the group A was 13.2%,while that in the group B was 5.4%,there was no significant difference in the total incidences of adverse reactions between the two groups(P>0.05).Conclusion The artificial femoral head replacement and PFNA in the treatment of intertrochanteric fracture of femur can achieve good clinical effects,with high safety.Operative methods can be selected according to different conditions of patients,and it is helpful for patients to recover as soon as possible.
