全血C-反应蛋白与白细胞计数联合检测在小儿肺炎鉴别诊断中的应用价值 被引量:3

摘要 目的探讨全血C-反应蛋白与白细胞计数联合检测在小儿肺炎检测中的效果。方法选取2017年1月~2018年12月我院收治的100例小儿肺炎患者作为研究对象,根据感染病原菌的差异将其分为细菌性肺炎组(n=33)、病毒性肺炎组(n=33)、支原体肺炎组(n=34),同期选取50例健康体检者作为对照组。研究对象均接受全血C-反应蛋白以及白细胞计数检测,比较C-反应蛋白、白细胞计数、中性粒细胞分类计数水平的差异。结果细菌性肺炎组的C-反应蛋白、白细胞计数、中性粒细胞分类计数水平均明显高于病毒性肺炎组、支原体肺炎组、对照组,支原体肺炎组的C-反应蛋白、白细胞计数、中性粒细胞分类计数水平均明显高于病毒性肺炎组和对照组,病毒性肺炎组的C-反应蛋白、白细胞计数、中性粒细胞分类计数水平均明显高于对照组,差异有统计学意义(P<0.05);细菌性肺炎组、病毒性肺炎组、支原体肺炎组的C-反应蛋白、白细胞计数、中性粒细胞分类计数联合阳性检出率均高于单独C-反应蛋白、白细胞计数、中性粒细胞分类计数的阳性检出率,差异有统计学意义(P<0.05)。结论在小儿肺炎鉴别诊断中实施全血C-反应蛋白与白细胞计数联合检测,可取得较好的临床效果。 Objective To investigate the effect of combined detection of whole blood C-reactive protein and white blood cell count in the detection of pneumonia in children.Methods A total of 100 children with pneumonia from the People′s Hospital of Nanhai Economic Development Zone from January 2017 to February 2018 were selected as subjects.According to the difference of contracted pathogens,they were divided into bacterial pneumonia group(n=33),viral pneumonia group(n=33),and mycoplasma pneumonia group(n=34).At the same time,50 healthy checker were selected as control group.All of them received whole blood C-reactive protein and white blood cell count detection.The difference of C-reactive protein,white blood cell count,and differential counting of neutrophil were compared.Results The C-reactive protein,white blood cell count and differential counting for neutrophil in the bacterial pneumonia group were significantly higher than those in the other three groups,the levels of C-reactive protein,white blood cell count,and differential counting of neutrophil in the mycoplasmal pneumonia group were significantly higher than those of the viral pneumonia group and the control group,the C-reactive protein,white blood cell count,and differential counting of neutrophil were significantly higher in the viral pneumonia group than in the control group with statistical significance(P<0.05).The positive combined detection rate of C-reactive protein,white blood cell count,and differential counting of neutrophil in the bacterial pneumonia,viral pneumonia and mycoplasma pneumonia groups were higher than those in detection of C-reactive protein,white blood cell count and differential counting of neutrophil differential count alone,the differences were statistically significant(P<0.05).Conclusion The combined detection of whole blood C-reactive protein and white blood cell count in the differential diagnosis of pediatric pneumonia can achieve favorable clinical effect.
