摘要
[目的]了解长期留置下腔静脉滤器病人的并发症与生活质量现状,并分析相关因素。[方法]采用一般资料问卷、Morisky服药依从性量表、慢性静脉功能不全生活质量量表并结合影像学检查结果,对126例长期留置下腔静脉滤器病人进行调研。[结果]本组病人滤器留置时间1~72个月,并发症发生率为37.30%,其中滤器内血栓形成19例(15.08%),下腔静脉血栓形成7例(5.56%),深静脉血栓(DVT)复发23例(18.25%),肺栓塞2例(1.59%),血栓后综合征12例(9.52%),无滤器断裂、倾斜、移位、下腔静脉穿孔等严重并发症;长期留置下腔静脉滤器病人生活质量得分为(81.15±6.05)分,53.17%的病人生活质量不理想(<80分);多元回归分析显示并发症、服药依从性、疾病相关知识、基础疾病是长期留置下腔静脉滤器病人生活质量的独立影响因素(P<0.05)。[结论]长期留置下腔静脉滤器病人并发症发生情况和生活质量不容乐观,服药依从性差的病人并发症发生几率更高。提示医务人员应进一步增强病人的疾病相关知识,提高服药依从性,从而减少并发症,改善生活质量。
Objective:To understand the complications and quality of life (QOL) of patients with long-term indwelling inferior vena cava(IVC)filter,and to analyze related factors.Methods:A total of 126 patients with long-term indwelling IVC filter were investigated with the General Information Questionnaire,Morisky Medication Adherence Scale(MMAS-8),Chronic Venous Insufficiency Questionnaire(CIVIQ-2),and imaging findings.Results:The retention time of the filter in this group was 1 to 72 months, and the complication rate was 37.30%.Among them,19 cases(15.08%)had thrombus formation in the filter,7 cases(5.56%) had IVC thrombosis,23 cases(18.25%)relapsed deep vein thrombosis(DVT),2 cases(1.59%)had pulmonary embolism,and 12 cases(9.52%)had post-thrombotic syndrome(PTS).There were no events of filter fracture,filter tilt,filter migration,IVC perforation,and other severe complications.The QOL of patients with long-term IVC filter was(81.15±6.05)points,and the QOL of 53.17% of patients was not ideal(<80 points).Multiple regression analysis showed that complications,medication compliance, disease-related knowledge,and basic diseases were the independent factors influencing the QOL of patients with long-term IVC filter(P<0.05).Conclusion:The complications and QOL of patients with long-term indwelling IVC filter were not optimistic, and the incidence of complications in patients with poor medication compliance was higher. It suggested that medical staff should further enhance the patient's disease-related knowledge,improve patients’ medication compliance,thereby reducing complications and improving their QOL.
