摘要
目的探讨血清甲状腺球蛋白(Tg)、促甲状腺激素(TSH)对分化型甲状腺癌的诊断价值。方法采用全自动电化学发光免疫分析法检测80例甲状腺结节患者的血清Tg、TSH、甲状腺过氧化物酶抗体(TPOAb)、促甲状腺激素受体抗体(TRAb)、甲状腺球蛋白抗体(TGAb)、甲状腺微粒体抗体(TMAb)水平及阳性率,根据病理检查结果确定病变的良恶性,以甲状腺细针穿刺病理检查结果为金标准,分析血清Tg、TSH对分化型甲状腺癌的诊断价值。结果甲状腺细针穿刺病理检查结果显示,80例甲状腺结节患者中,分化型甲状腺癌患者40例,甲状腺良性结节患者40例。分化型甲状腺癌患者的血清TSH、TPOAb、TRAb、TGAb、TMAb水平及阳性率均高于甲状腺良性结节患者,差异均有统计学意义(P﹤0.05)。TSH检测分化型甲状腺癌的灵敏度、特异度和准确度均较高,Tg检测分化型甲状腺癌的灵敏度、特异度和准确度均较低。结论与Tg相比,TSH对分化型甲状腺癌诊断的准确度、灵敏度和准确度均较高,TSH可作为临床诊断分化型甲状腺癌的重要指标。
Objective To investigate the diagnostic value of serum thyroglobulin(Tg)and thyroid stimulating hormone(TSH)in differentiated thyroid carcinoma.Method The serum levels of Tg,TSH,thyroid peroxidase antibody(TPOAb),TSH receptor antibody(TRAb),thyroglobulin antibody(TGAb)and thyroid microsome antibody(TMAb)and their positive rates in eighty patients with thyroid carcinoma or thyroid nodules were analyzed using electrochemiluminescence assay.The benign and malignant lesions were determined according to the pathological examination results,and the gold standard was the thyroid fine needle(FNA)aspiration biopsies puncture pathological examination results to analyze the diagnostic value of serum Tg and TSH in differentiated thyroid carcinoma.Result The FNA aspiration biopsies puncture pathological examination results revealed that,among the 80 cases with thyroid nodules,there were 40 cases of differentiated thyroid carcinoma,and 40 cases with benign thyroid nodules.The serum levels of TSH,TPOAb,TRAb,TGAb and TMAb and the positive rates in the differentiated thyroid carcinoma were significantly higher than those in the benign thyroid nodules,the differences were of statistical significance(P<0.05).TSH has a high sensitivity,specificity and accuracy in detecting differentiated thyroid carcinoma,while Tg has a low sensitivity,specificity and accuracy in detecting differentiated thyroid carcinoma.Conclusion Compared with Tg,TSH has higher accuracy,sensitivity and accuracy in the diagnosis of differentiated thyroid cancer,and TSH can be used as an important indicator for the clinical diagnosis of differentiated thyroid cancer.
