摘要
随着国家二胎政策的开放,使许多家庭又点燃孕育的希望,为此高龄产妇人数大幅提升,难孕、不孕、难产人群比重明显增加[1],子宫内膜过薄是导致胚胎种植失败的主要原因,目前影响子宫内膜相关因子研究较多的有EMX2基因、整合素β3、HOXA10基因、vimentin、CK、CD31、TNF-α、IL-1等,对于提高子宫内膜容受性目前多从促进内膜修复、改善内膜血流灌注、降低血液黏稠度、增加组织氧分压、提升子宫内膜细胞的修复和再生能力等为主要突破口。尽管薄型子宫内膜治疗靶点的选择及阐述机制多种多样,但其疗效并不容乐观,大多中医专家认为肾虚血瘀是其产生的主要原因,故以补肾活血为基本大法,在改善子宫内膜厚度、提高妊娠率及活产率方面发挥了独特优势。本文通过新近多种改善子宫内膜厚度及内膜再生情况的解决策略,意在发现更有优势的治疗导向并指导于临床。
With the opening of the national policy of second child, many families have lit up the hope of gestation. For this reason, the number of elderly women has greatly increased, the proportion of difficult pregnancy, infertility, and dystocia has increased significantly. But the thinning of the endometrium is the main cause of the failure of embryo implantation. Currently factors related to endometrium EMX2 gene, integrin β3, HOXA10 gene, vimentin, CK, CD31, TNF-α, IL-1, etc. To improve the endometrial receptivity at present from promote endometrial repair, improve endometrial perfusion, reduce blood viscosity, increase tissue oxygen pressure, improve endometrial cell repair and regeneration ability as the main breakthrough. Although thin endometrium therapeutic target selection and elaborate mechanism varied, but its effect is not optimistic most experts believe that the traditional Chinese medicine, kidney deficiency and blood stasis are the main reasons, so as the basic method of invigorating kidney and promoting blood circulation, improve endometrial thickness, play a unique advantage to improve the pregnancy rate and live birth rate. We improve the endometrial thickness and the regeneration of a variety of solutions through the review of recent, intended to find more advantage of the treatment guide and to guide clinical practice.
