摘要
目的通过对门急诊处方的点评分析,找出存在的问题,促进临床合理用药.方法每月随机抽取门急诊处方300张进行点评,汇总和分析2017年7-12月不合理处方中关于用药方面存在的常见问题.结果 1800张处方中合理处方1636张(90.89%).不合理处方164张(9.11%),不合理处方存在的主要问题为给药频次不适宜、给药剂量不适宜、给药途径不适宜、选药不适宜及药品超常联用.结论处方质量关系到医院的医疗质量和医疗安全,处方点评是促进临床安全、合理用药的重要手段.
Objective To find out the problems of prescriptions and evaluate the rational use of drugs,through the evaluation of prescriptions in outpatient and emergency department. Methods 300 prescriptions in outpatient and emergency department were randomly selected for review,and the common problems in the use of medicines in the unreasonable prescriptions from July to December 2017 were summarized and analyzed. Results 1 636( 90. 89%) reasonable prescriptions in 1 800 prescriptions. Unreasonable prescription of 164( 9. 11%),the main problems of unreasonable prescription include im- proper interval of drug administration,improper administration dosage,improper administration route,incorrect drug selection and drug supernormal combination. Conclusion The quality of prescription is crucial to medical quality and medical safety of our hospital,while prescription review is an important means to promote safe and rational use of drugs.
