血液透析联合血液灌流治疗尿毒症脑病效果及对患者血清脑源性神经营养因子和神经肽Y的影响 被引量:17

Effect and changes of BDNF and NPY in patients with uremic encephalopathy by hemodialysis combined hemoperfusion

摘要 目的:探讨血液透析(HD)联合血液灌流(HP)对尿毒症脑病的治疗效果,同时观察血清中脑源性神经营养因子(BDNF)和神经肽Y(NPY)含量的变化。方法:选择94例尿毒症脑病患者,随机分为两组,每组各47例,对照组应用HD治疗,观察组应用HD+HP治疗。分别于治疗前、后检测血清中血尿素氧(BUN)、血肌酐(Scr)、全段甲状旁腺激素(iPTH)、β2-微球蛋白(β2-MG)、BDNF和NPY的水平。结果:观察组的总有效率80.9%明显高于对照组的61.7%(χ2=4.209,P<0.05),两组治疗后BUN、Scr、iPTH和β2-MG均下降,但是观察组的下降值更明显。两组治疗后BDNF和NPY均升高,但是观察组的升高值更明显。结论:尿毒症脑病患者应用HD+HP治疗的效果明显,对清除尿毒症毒素,提高治疗效果有明显价值,同时能调节血清中BDNF和NPY的含量。 Objective:To investigate the therapeutic effect,changes of serum brain-derived neurotrophic factor(BDNF) and neuropeptide Y(NPY) in patients with uremic encephalopathy by hemodialysis(HD) combined with hemoperfusion(HP).Methods:94 patients with uremic encephalopathy were divided into two groups according to the order of admission(47 cases of each group). The control group was treated with HD, the observation group was treated with HD and HP. Serum levels of BUN, Scr, iPTH, beta2-microglobulin(beta2-MG), BDNF and NPY were detected before and after treatment. Results: The total effective rate was higher in the observation group than that of the control group. BUN, Scr, iPTH and β2-MG were decreased after treatment in two groups, but the decrease value of observation group was more obvious. BDNF and NPY were increased after treatment in two groups, but the increased value of the observation group was more obvious. Conclusion: HD and HP is effect to the treatment of uremic encephalopathy. It has obvious value in clearing uremic toxin and improving therapeutic effect. It can also regulate the content of BDNF and NPY, Should be used actively in clinical treatment.
