摘要
目的探讨不同手术方式治疗急性闭角型青光眼合并白内障的疗效。方法78例(78只眼)急性闭角型青光眼合并白内障患者按随机数字表法分为A组和B组,A组采用白内障超声乳化联合人工晶体植入术治疗,B组采用白内障超声乳化联合人工晶体植入+小梁切除术治疗,对比观察两组手术前后视力、眼压变化情况以及术后并发症的发生情况。结果两组术后视力均较术前明显提高,术前与术后比较差异有统计学意义(P<0.05),两组术后视力比较差异无统计学意义;两组术后眼压较术前均显著降低,术前与术后比较差异有统计学意义(P<0.05),两组术后眼压比较差异无统计学意义。两组并发症发生率组间比较差异无统计学意义。结论对于急性闭角型青光眼合并白内障患者而言,白内障超声乳化联合人工晶体植入术和白内障超声乳化联合人工晶体植入+小梁切除术均能有效提高视力并且控制好眼压,而且并发症发生率低,手术安全。
Objective To investigate the effect of different surgical methods in the treatment of acute angle-closure glaucoma and cataract.Methods 78 patients(78 eyes)with acute angle closure glaucoma and cataract were randomly divided into group A and group B,the patients in group A received phacoemulsification combined with intraocular lens implantation,while the patients in group B received phacoemulsification combined with intraocular lens implantation plus trabeculectomy,the changes of vision and intraocular pressure before and after operation and the complications of both groups were observed.Results Postoperative visual acuity in both groups were significantly improved compared with preoperative,there was significant difference between preoperative and postoperative(P<0.05),while there was no significant difference between the two groups in the comparison of the postoperative visual acuity;the postoperative intraocular pressure of the two groups were significantly lower than preoperative,there was significant difference between preoperative and postoperative(P<0.05),while there was no significant difference between the two groups in the comparison of the postoperative intraocular pressure.There was no significant difference in the incidence of complications between the two groups.Conclusion For patients with acute angle closure glaucoma and cataract,phacoemulsification combined with intraocular lens implantation and phacoemulsification combined with intraocular lens implantation plus trabeculectomy can both effectively improve vision and control the intraocular pressure,and the incidence of complications is low.
