摘要
妊娠妇女子宫的免疫微环境不仅要保护胎儿和母体免受外界病原体的威胁,还要维持母体对胎儿体内父源性的同种异型抗原的免疫耐受,免疫系统的失衡会引起流产、早产、子痫前期等妊娠并发症的发生。辅助性T细胞(Th)Th1与Th2细胞亚群在妊娠免疫平衡状态维持中的作用曾经是子痫前期发病机制的研究热点。近年来研究发现Th17细胞与调节性T细胞亚群在子痫前期患者体内均有比例及功能的改变,并受复杂的细胞及分子机制调节,可能在子痫前期发病机制中具有更重要的作用。本文将就Th17与Treg细胞在子痫前期中的作用及调控机制的最新进展加以综述,以期促进对子痫前期免疫学发病机制的阐明及诊断治疗策略的开发。
The immune microenvironment of pregnant women′s uterus not only protects the fetus and the mother from the threat of external pathogens,but also maintains the maternal immune tolerance to the paternal allotype antigens in the fetus.Imbalance of the immune system will cause miscarriage,premature birth,preeclampsia and other pregnancy complications.Roles of helper T cell(Th)Th1 cell subsets and Th2 cell subsets in the maintenance of pregnancy immune balance used to be a hot topic in the pathogenesis of preeclampsia.In recent years,it has been found that Th17 cells and regulatory T cell(Treg)subsets have great changes of ratios and functions in most of the patients with preeclampsia.These cell subsets are regulated by complex cellular and molecular mechanisms,which may be more important in the pathogenesis of preeclampsia.This article will review the recent advances in the role and regulation of Th17 and Treg cells in preeclampsia and aim to promote the elucidation of the pathogenesis of preeclampsia and the development of diagnostic strategies.
