摘要
目的 探讨血浆肌肉因子鸢尾素(Irisin)含量与肌肉减少症的相关性。方法 选取2016年3月至2018年3月在河南省人民医院体检中心50岁以上的体检者580例作为受试者,其中肌肉减少症者237例(试验组),非肌肉减少症者343例(对照组);为避免性别影响,将试验组分为男性试验组和女性试验组,对照组分为男性对照组和女性对照组。记录受试者的一般情况。根据肌肉减少症的严重程度,将试验组分为肌肉减少症前期、肌肉减少症中期和肌肉减少症严重期3组。用酶联免疫吸附测定(enzyme linked immunosorbent assay, ELISA )法检测受试者血浆中Irisin的含量。统计分析各组间Irisin含量的差异,Pearson相关分析Irisin含量与受试者肌肉减少程度的相关性。并绘制血浆Irisin含量诊断肌肉减少症程度的受试者工作曲线。结果 试验组和对照组比较分析年龄、体质量指数(body mass index,BMI)、腹部骨骼肌、臀部骨骼肌、脂肪含量(fat mass,FM)、臀部FM、全身骨骼肌含量、四肢骨骼肌含量、相对骨骼肌指数(relative appendicular skeletal muscle mass,RASM)、握力、步速、Irisin差异有统计学意义( P =0.000);试验组间对比:肌肉减少症前期Irisin含量最高(男性: M =198.73 μg/L;女性: M = 217.70 μg/L ),肌肉减少症严重期含量最低(男性: M =145.65 μg/L;女性: M =187.75 μg/L),不同性别组内差异有统计学意义(男性: Z =-13.725 , P =0.000;女性: Z =-6.992, P =0.000)。Pearson相关分析表明Irisin含量与相对骨骼肌指数RASM均呈正相关(男性: r =0.846, P =0.000;女性: r =0.488, P =0.000);ROC曲线显示男性血浆Irisin含量 226.425 μg/L是诊断最优截断值,其诊断肌少症的敏感度为 0.929,特异度为 0.943,Youden指数为0.871;女性血浆Irisin含量为226.865 μg/L是诊断最优截断值,其诊断肌少症的敏感度为 0.808,特异度为 0.673,Youden指数为0.481。结论 随着肌少症程度增加患者血浆Irisin含量越低,临�
Objective To investigate the correlation between plasma irisin level and sarcopenia. Methods A total of 580 subjects aged over 50 were selected from population, who received physical examination in Henan Provincial People s Hospital from 2016 to 2018. 237 with had sarcopenia were divided into test group and 343 without sarcopenia were as control group. The general condition of the subjects was recorded. According to the severity of sarcopenia,the subjects were divided into three groups: prophase,metaphase and severe phase of sarcopenia. The plasma content of Irisin was detected by enzyme linked immunosorbent assay(ELISA). Irisin content differences between the groups were analysed.Pearson correlation analysis was completed to investigate Irisin content and the degree of muscle loss. The receiver operating curve of plasma Irisin level was used to diagnose the degree of muscle reduction. Results There were significant differences in age,body mass index(BMI),android skeletal muscle,fat mass(FM),gynoid FM,gynoid skeletal muscle,total skeletal muscle content,limb skeletal muscle content,relative appendicular skeletal muscle mass(RASM),grip strength,walking speed and Irisin between the male test group and the control group ( P =0.000). The Irisin content was the highest in prophase of sarcopenia (male: M= 198.73 μg/L,female: M =217.70 μg/L) and the lowest in severe phase of sarcopenia (male: M =145.65 μg/L,female: M =187.75 μg/L).There was significant difference between different gender groups (male: Z =-13.725, P =0.000;female: Z =-6.992, P =0.000). Pearson correlation analysis showed that Irisin content was positively correlated with RASM (male: r =0.846, P = 0.000;female: r =0.488, P =0.000). The ROC curve showed that the best cutoff value of Irisin content in men was 226.425 μg/L,and the sensitivity,specificity and Youden index were 0.929,0.943 and 0.871 respectively.
