摘要
目的分析医院门诊退药处方的原因,并提出相应的防范措施。方法收集该院门诊药房在2017年1月—2018年10月所发生的78例退药事件,对所有退药申请的单据进行了整理分析,针对其中退药药品种类、退药科室、退药的原因等进行了详细的分析,并制定出了相应的防范措施。结果78例退药事件中有38例(48.72%)为药品因素;有21例(26.92%)为医师因素;有19例(24.36%)为患者因素,各组之间差异有统计学意义(P<0.05)。结论导致医院门诊药房退药的因素有很多,这就要求临床医师、药剂师均能够对影响门诊药房退药的原因进行分析,将门诊药房退药率降到最低,促进门诊药房药物的合理使用。
Objective To analyze the causes of drug withdrawal prescriptions in hospital outpatients and propose corresponding preventive measures. Methods 78 drug withdrawal incidents occurred in the outpatient pharmacy of our hospital from January 2017 to October 2018 were collected. The documents for all drug withdrawal applications were analyzed,and the types of drug withdrawals,drug withdrawal departments,and withdrawals reasons for the drug were analyzed in detail,and corresponding preventive measures were developed. Results 38 cases (48.72%) of the 78 drug withdrawal events were drug factors;21 cases (26.92%) were physician factors;19 cases (24.36%) were patient factors,and the differences between the groups were significant being statistically significant (P<0.05). Conclusion There are many factors leading to drug withdrawal in hospital outpatient pharmacies. This requires clinicians and pharmacists to analyze the causes of drug withdrawals in outpatient pharmacies,minimize the drug withdrawal rate in outpatient pharmacies,and promote the rational use of drugs in outpatient pharmacies.
