骨水泥强化空心侧孔椎弓根螺钉在退行性腰椎手术中的应用研究 被引量:1

摘要 目的探讨骨水泥强化空心侧孔椎弓根螺钉在退行性腰椎手术中的应用效果。方法选取2016年9月至2018年6月郑州大学第二附属医院收治的62例退行性腰椎手术患者,将其随机分为研究组(空心侧孔螺钉强化)与对照组(常规椎弓根钉道强化),每组31例。评估两组手术时间,总出血量(术中+术后),VAS评分、ODI评分,并发症及复查随访情况。结果研究组手术时间[(122.19±7.84)min]短于对照组[(134.26±8.98)min],两组总出血量比较差异未见统计学意义(P>0.05)。两组VAS、ODI评分均较术前显著改善(P<0.05),研究组骨水泥渗漏及并发症发生率低于对照组(P<0.05)。结论空心侧孔骨水泥加强螺钉技术可改善患者症状,减少手术时间及并发症,值得临床应用。 Objective To investigate the effect of cement-reinforced fenestrated pedicle screw on degenerative lumbar surgery. Methods A total of 62 patients with degenerative lumbar surgery in the Second Affiliated Hospital of Zhengzhou University from September 2016 to June 2018 were randomly divided into study group(fenestrated screw fixation)and control group(conventional pedicle screw fixation), with 31 cases in each group. The operation time and total of intraoperative and postoperative bleeding volume, visual analogue scale (VAS) score, Oswestry disability index (ODI) score, complication rate and follow-up were evaluated. Results The operation time of the study group [(122.19±7.84) min] was shorter than that of the control group[(134.26±8.98) min]. There was no significant difference in the total amount of bleeding between the two groups(P>0.05). The VAS and ODI scores of the two groups were significantly improved compared with the preoperative scores (P<0.05). The incidences of bone cement leakage and complications in the study group were lower than those in the control group (P<0.05). Conclusions The technique of fenestrated cement-reinforced screws can improve the symptoms, shorten the operation time and reduce the complications, which is worthy of clinical application.
