摘要
目的:观察体外开窗治疗累及左锁骨下动脉Stanford B型主动脉夹层的近期疗效.方法:回顾性分析我院2016年11月-2017年10月收治的17例累及左锁骨下动脉Stanford B型主动脉夹层患者,均采用体外开窗胸主动脉腔内修复术(TE-VAR)治疗,随访至2018年9月,观察患者近期疗效.结果:患者平均手术时间(58.2±14.6)分钟,术后平均住院时间(5.9±1.2)天.放置覆膜支架18个,术中支架全部释放成功,成功率100%.围术期并发症发生率为11.76%,无1例患者围术期发生急性脊髓缺血、脑血管事件、脊髓损伤等严重并发症,也无1例患者死亡.患者均获得随访,平均随访(12.7±2.1)个月,支架封堵良好,夹层假腔均出现血栓化.末次随访主动脉增强CT复查显示,患者支架完好无移位,均恢复正常生活.结论:体外开窗治疗累及左锁骨下动脉Stanford B型主动脉夹层的近期疗效肯定,并发症发生率低,中远期尚在继续随访.
Objective:To observe the short-term efficacy of external fenestration in the treatment of involvement of Stanford b-type aortic dissection of the left subclavian artery.Methods:A retrospective analysis was performed on 17 cases of Stanford B type aortic dissection involving the left subclavian artery in our hospital from November 2016 to October 2017.All of them were treated with external window open thoracic aortic cavity repair(TEVAR),followed up until September 2018,and the short-term efficacy was observed.Results:Mean operative time(58.2±14.6)min,mean postoperative hospital stay(5.9±1.2)d.There were 18 covered stents,all of which were successfully released during the operation,with a success rate of 100%.The incidence of perioperative complications was 11.76%,but none of the patients had severe complications such as acute spinal ischemia,cerebrovascular events,and spinal cord injury,or died.All the patients were followed up,with an average follow-up(12.7±2.1)month.The stents were well blocked,and the dissected pseudocavities all presented thrombosis.The final follow-up aortic enhancement CT examination showed that the stent was intact without displacement,and all returned to normal life.Conclusion:External fenestration in the treatment of the involvement of Stanford B type aortic dissection of the left subclavian artery had a positive short-term effect,with a low incidence of complications and a long term follow-up.
