摘要
目的研究脾切除加断流术治疗晚期血吸虫病门脉高压症的效果。方法选取在我院2015年2月-2018年12月收治的晚期血吸虫病门脉高压症患者中选出280例作为研究对象,均在我院接受脾切除+断流术治疗,收集疗效指标。结果本组280例患者均手术成功,其中5例患者术中中转开腹手术治疗,术后血小板计数和白细胞计数均有不同程度的提高,术后1~2周逐渐降低至正常水平。术后2周复查肝功能Child分级:A级232例,B级45例,C级3例。术后并发症发生率3.93%,未有死亡病例。结论脾切除+断流术在晚期血吸虫病门脉高压症治疗中效果确切,促进患者肝功能改善,并发症少,提高患者的生活质量。
Objective To study the effect of splenectomy plus devascularization in the treatment of advanced schistosomiasis with portal hypertension.Methods A total of 280 patients with advanced schistosomiasis portal hypertension admitted to our hospital from February2015 to December 2018 were enrolled in our hospital for splenectomy and devascularization.Results All the 280 patients in this group were successfully operated,among which 5 patients were transferred to open surgery during the operation,the postoperative platelet count and white blood cell count were increased to varying degrees,and gradually decreased to the normal level 1~2 weeks after the operation.Two weeks after surgery,liver function Child grades were reviewed:232 cases of grade A,45 cases of grade B,and 3 cases of grade C.The postoperative complication rate was 3.93%,and there were no deaths.Conclusion Splenectomy plus devascularization is effective in the treatment of advanced schistosomiasis with portal hypertension,which can improve the liver function,reduce complications and improve the quality of life of patients.
