摘要
目的:探讨超敏肌钙蛋白T(hs-TnT)、血浆脂蛋白a(Lpa)、同型半胱氨酸(Hcy)联合检测在诊断冠状动脉粥样硬化性心脏病(CHD)中的应用价值。方法:以98例CHD患者为观察组,根据是否合并心肌梗死又分为心肌梗死组和无心肌梗死组,另选取同期行体格检查的85例健康志愿者为对照组,采用电化学发光法检测hs-TnT水平,用循环酶法检测Hcy水平,以透射免疫浊度法检测Lpa水平。结果:观察组hs-TnT、Lpa和Hcy水平明显高于对照组,且心肌梗死组显著高于无心肌梗死组(均P<0.01)。观察组中,hs-TnT、Lpa和Hcy三者联合检测的阳性率为91.84%,明显高于hs-TnT、Lpa和Hcy单独检测(69.39%、68.37%和62.24%,P<0.01)。结论:CHD患者hs-TnT、Lpa和Hcy水平明显升高,且合并心肌梗死者高于未合并心肌梗死者;hs-TnT、Lpa和Hcy三者联合检测可提高CHD的检出率,对诊断CHD有重要的应用价值。
Objective: To evaluate the diagnostic value of combined detection of hypersensitive troponin T (hs-TNT),plasma lipoprotein a (Lpa) and homocysteine (Hcy) in the diagnosis of coronary heart disease (CHD). Methods: A total of 98 patients with CHD in our hospital from July 2015 to January 2017 were enrolled as an observation group,and 85 healthy subjects were defined as a control group.The serum levels of hs-TNT,Hcy and Lpa were measured by the methods of electrochemical luminescence,enzymatic cycling and transmission immune turbidimetry,respectively. Results: The hs-TnT,Lpa and Hcy levels in the observation group were significantly higher than those in the control group,and the patients with myocardial infarction had higher levels of hs-TnT,Lpa and Hcy than those without ( P <0.01).The combined detection rate of hs-TnT,Lpa and Hcy was 91.84%,and it was higher than hs-TnT,Lpa and Hcy detection alone (69.39%,68.37% and 62.24%)( P <0.01). Conclusion: The levels of hs-TnT,Lpa and Hcy in patients with CHD were significantly increased,and were higher in patients with myocardial infarction than those without.The combined detection of hs-TnT,Lpa and Hcy was more valuable for the diagnosis of CHD.
