超敏肌钙蛋白T、血浆脂蛋白a和同型半胱氨酸联合检测在冠状动脉粥样硬化心脏病诊断中的应用价值 被引量:14

摘要 目的:探讨超敏肌钙蛋白T(hs-TnT)、血浆脂蛋白a(Lpa)、同型半胱氨酸(Hcy)联合检测在诊断冠状动脉粥样硬化性心脏病(CHD)中的应用价值。方法:以98例CHD患者为观察组,根据是否合并心肌梗死又分为心肌梗死组和无心肌梗死组,另选取同期行体格检查的85例健康志愿者为对照组,采用电化学发光法检测hs-TnT水平,用循环酶法检测Hcy水平,以透射免疫浊度法检测Lpa水平。结果:观察组hs-TnT、Lpa和Hcy水平明显高于对照组,且心肌梗死组显著高于无心肌梗死组(均P<0.01)。观察组中,hs-TnT、Lpa和Hcy三者联合检测的阳性率为91.84%,明显高于hs-TnT、Lpa和Hcy单独检测(69.39%、68.37%和62.24%,P<0.01)。结论:CHD患者hs-TnT、Lpa和Hcy水平明显升高,且合并心肌梗死者高于未合并心肌梗死者;hs-TnT、Lpa和Hcy三者联合检测可提高CHD的检出率,对诊断CHD有重要的应用价值。 Objective: To evaluate the diagnostic value of combined detection of hypersensitive troponin T (hs-TNT),plasma lipoprotein a (Lpa) and homocysteine (Hcy) in the diagnosis of coronary heart disease (CHD). Methods: A total of 98 patients with CHD in our hospital from July 2015 to January 2017 were enrolled as an observation group,and 85 healthy subjects were defined as a control group.The serum levels of hs-TNT,Hcy and Lpa were measured by the methods of electrochemical luminescence,enzymatic cycling and transmission immune turbidimetry,respectively. Results: The hs-TnT,Lpa and Hcy levels in the observation group were significantly higher than those in the control group,and the patients with myocardial infarction had higher levels of hs-TnT,Lpa and Hcy than those without ( P <0.01).The combined detection rate of hs-TnT,Lpa and Hcy was 91.84%,and it was higher than hs-TnT,Lpa and Hcy detection alone (69.39%,68.37% and 62.24%)( P <0.01). Conclusion: The levels of hs-TnT,Lpa and Hcy in patients with CHD were significantly increased,and were higher in patients with myocardial infarction than those without.The combined detection of hs-TnT,Lpa and Hcy was more valuable for the diagnosis of CHD.
