摘要
目的:研究环氧化酶(COX)抑制剂氟比洛芬酯预先给药对切口痛大鼠镇痛的中枢机制。方法:将100只SD大鼠随机分为模型组、低剂量组、正常剂量组、高剂量组,每组25只。各组均复制大鼠切口痛模型,建模前30min,低剂量组、正常剂量组和高剂量组分别于尾静脉注射3mg/kg、6mg/kg、12mg/kg氟比洛芬酯。术前及术后1h、6h、12h,测定各组大鼠左后爪的机械刺激缩足反应阈值(PMWT)和热缩足潜伏期(TWL)。采用酶联免疫吸附试验(ELISA)法测定血清白细胞介素(IL)-6、IL-10及血清、脊髓和下丘脑弓状核(ARC)中β-内啡肽水平。免疫组织化学染色法检测脊髓和ARC中β-内啡肽的表达水平。结果:术后1h、6h、12h,用药组PMWT、血清IL-6及血清和脊髓中的β-内啡肽水平均明显低于模型组,TWL、血清IL-10及ARC中β-内啡肽水平明显高于模型组(P<0.05)。高剂量组和正常剂量组PMWT、TWL显著高于低剂量组,且高剂量组更为显著(P<0.05)。高剂量组术后不同时间点血清IL-6水平显著低于正常剂量组和低剂量组,而IL-10和血清、脊髓及ARC中β-内啡肽水平显著高于正常剂量组和低剂量组(P<0.05)。术后不同时间点,高剂量组脊髓和ARC中β-内啡肽阳性细胞数明显增加。结论:COX抑制剂氟比洛芬酯预先给药对切口痛大鼠有良好的镇痛作用,其作用机制可能与降低炎症反应、提高中枢β-内啡肽水平有关。
Objective: To investigate the central mechanism of flurbiprofen axetil,a cyclooxygenase (COX) inhibitor,on incisional pain in rats. Methods: A total of 100 SD rats were randomly divided into model group,low-dose (3 mg/kg) group,normal-dose (6 mg/kg) group and high-dose (12 mg/kg) group,with 25 rats in each group.The low-,normal- and high-dose group rats were given flurbiprofen axetil via tail vein.A incisional pain rat model was established before 30 min of treatment.The paw withdrawal mechanical threshold (PMWT) and thermal withdrawal latency (TWL) were assessed before and after 1 h,6 h and 12 h of surgery.The serum levels of interleukin (IL)-6,IL-10 and β-endorphine as well as the β-endorphine in spinal cord and hypothalamic arcuate nucleus (ARC) were detected by enzyme linked immunosorbent assay (ELISA).The expression of β-endorphin in spinal cord and ARC was determined by immunohistochemical staining. Results: The PMWT,serum IL-6 level,and β-endorphin content in serum and spinal cord were significantly lower in the medication group than those in the model group,while the β-endorphin in ARC,TWL and IL-10 were higher ( P <0.05).The PMWT and TWL in the normal-dose group and high-dose group were significantly higher than those in the low-dose group,and the change was more marked in high-dose group ( P <0.05).The level of IL-6 in high-dose group was significantly lower than that in normal-dose group and low-dose group at different time points after operation,and serum IL-10 level and β-endorphin level in serum,spinal cord and ARC were significantly higher ( P <0.05).These changes were also more significant in high-dose group.After operation,the number of β-endorphin positive cells in spinal cord and ARC in high-dose group was significantly increased. Conclusion: Pre-treatment of the COX-2 inhibitor flurbiprofen axetil exerted a good analgesic effect in incisional pain rats,and the mechanisms might be related to the inhibition of inflammatory reaction and elevation of β-endorphine level in the central nerve
