阿奇霉素联合小剂量糖皮质激素治疗儿童大叶性肺炎的临床疗效 被引量:6

摘要 目的:探讨阿奇霉素联合小剂量糖皮质激素治疗儿童大叶性肺炎的临床疗效。方法:选择2016年7月至2018年12月期间医院收治的儿童大叶性肺炎患儿80例,随机数字表法分组,对照组40例接受阿奇霉素治疗,观察组40例接受阿奇霉素联合小剂量糖皮质激素治疗。比较两组治疗前后肺炎实验室指标,症状消失时间及疗效等。结果:观察组治疗疗效95%,高于对照组的65%,差异有统计学意义(P<0.05)。观察组住院时间、X线片恢复正常时间、啰音消失时间、咳嗽持续时间、发热消退时间均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组ESR、CRP低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组FEF25%高于对照组,差异有统计学意义(P<0.05)。两组FEV1、FVC比较,差异无统计学意义。结论:阿奇霉素联合小剂量糖皮质激素治疗儿童大叶性肺炎,可显著改善其炎性指标,改善患儿的肺功能,具有良好的临床疗效和安全性。 Objective:To investigate the clinical efficacy of azithromycin combined with low-dose glucocorticoids in the treatment of children with lobar pneumonia.Methods:Eighty children with lobar pneumonia admitted to hospital from July 2016 to December 2018 were enrolled in the hospital.Randomized digital table was used.40 patients in the control group received azithromycin treatment,and 40 patients in the observation group received azithromycin combined with low-dose sugar.Corticosteroid treatment.The laboratory parameters of pneumonia before and after treatment,the time of disappearance of symptoms and the curative effect were compared.Results:The therapeutic effect of the observation group was 95%,which was higher than that of the control group (65%).The difference was statistically significant ( P < 0.05 ).The hospitalization time,the normal time of X-ray film recovery,the disappearance time of the voice,the duration of cough,and the time of fever regression were lower in the observation group than in the control group,and the difference was statistically significant ( P <0.05).After treatment,ESR and CRP in the observation group were lower than those in the control group,and the difference was statistically significant ( P <0.05).After treatment,the FEF of the observation group was 25% higher than that of the control group,and the difference was statistically significant ( P <0.05).There was no significant difference between the two groups of FEV1 and FVC.Conclusions:Azithromycin combined with low-dose glucocorticoid therapy for children with lobar pneumonia can significantly improve its inflammatory index and improve lung function in children,with good clinical efficacy and safety.
