摘要
目的探讨血清同型半胱氨酸(homocysteine,Hcy)以及载脂蛋白(apolipoprotein,Apo) A-Ⅰ水平与冠心病以及冠状动脉病变程度之间的关系。方法选择2016年11月至2018年2月宣城市人民医院经冠状动脉造影诊断为冠心病的病人168例,对照选择同期在该院检查排除冠心病的人群179例。血清Hcy采用循环酶法测定,血清Apo A-Ⅰ采用美国BECKMAN DX-800型生化分析仪测定。两变量间的相关性以Spearman秩相关系数(rs)表示。采用二分类logistic回归分析血清Hcy及Apo A-Ⅰ与冠心病风险之间的独立关联。结果冠心病组病人血清Hcy水平[M(P25,P75)]为14.18(10.99,15.43)μmol/L,显著高于对照组9.03(8.26,9.70)μmol/L,Z=-11.69,P<0.001。冠心病组病人血清Apo A-Ⅰ水平[M(P25,P75)]为1.15(0.88,1.42)g/L,显著低于对照组1.64(1.35,1.82) g/L,Z=-9.81,P<0.001。二分类logistic回归分析结果显示:血清Hcy水平每增加1个三分位等级,冠心病发病风险的OR值为2.39(95%CI:1.53~3.70);血清Apo A-Ⅰ水平每增加1个三分位等级,冠心病发病风险的OR值为0.79(95%CI:0.65~0.96)。相关分析结果显示:血清Hcy均与血管病变支数及SYNTAX评分呈正相关关系(rs=0.252,P<0.05),Apo A-Ⅰ与血管病变支数呈负相关(rs=-0.257,P<0.05)。结论血清Hcy水平与冠心病风险及冠脉病变程度呈负相关,Apo A-Ⅰ水平与冠心病风险及冠脉病变程度呈正相关。
Objective To investigate the association of the level of serum homocysteine(Hcy) and apolipoprotein A-Ⅰ(Apo A-Ⅰ) with risks of coronary heart disease(CHD) and the severity of coronary artery lesions.Methods A total of 168 cases of CHD diagnosed by coronary arteriography in The People’s Hospital of Xuancheng City from November 2016 to February 2018 were recruited in this study.And 179 cases of controls were selected from these who were free of coronary heart disease in the hospital during the same period.Serum Hcy was determined by cyclic enzymatic method,and the serum Apo A-Ⅰ was determined by BECKMAN DX-800 biochemical analyzer.The correlation between the two variables was expressed by Spearman rank correlation coefficient(rs).Results There was no significant difference in characteristics of age and gender between CHD and control group.The level of serum Hcy in the CHD group was14.18(10.99,15.43)μmol/L,which was significantly higher than that in the control group 9.03(8.26,9.70)μmol/L(Z=-11.69,P<0.001).The serum Apo A-Ⅰ level 1.15(0.88,1.42) g/L in patients with CHD was significantly lower than that in the control group 1.64(1.35,1.82) g/L,(Z=-9.81,P<0.001).The binary logistic regression analysis indicated that the OR of the risk of CHD was 2.39(95% CI:1.53-3.70) for per tertile increase in serum Hcy,and the OR of the risk of CHD was 0.79(95% CI:0.65-0.96) for per tertile increase in serum Apo A-Ⅰ.The correlation analysis results showed that serum Hcy was positively correlated with the number of vascular lesions and the scores of SYNTAX(rs=0.252,P<0.05),and Apo A-Ⅰ was negatively correlated with the number of vascular lesions(rs=-0.257,P<0.05).Conclusion The level of serum Hcy is negatively correlated with the risk of coronary heart disease and the degree of coronary artery disease.The level of Apo A-Ⅰ is positively associated with coronary heart disease and the degree of vascular lesions.
