摘要
目的探讨高流量装置对撤机后人工气道留置时间的影响。方法收集2017年7~12月期间于本院ICU住院进行人工气道留置及机械通气(mechanical ventilation,MV)的394例患者为观察组,2017年1~6月期间于本院ICU住院进行人工气道留置的357例患者为对照组,对2组人工气道留置及MV的患者进行评估,达到脱机指征的患者进行30~120min的自主呼吸试验(spontaneous breathing trial,SBT)和气囊漏气试验,观察组使用高流量吸氧装置,对照组使用T管吸氧。比较2组患者撤机后30min的pH、PaO2、PaCO2、RR、氧合指数(PaO2/FiO2)血气分析指标以及人工气道留置时间。结果观察组撤机后30min的pH、PaO2、PaCO2、RR、氧合指数(PaO2/FiO2)血气分析指标显著高于对照组(P<0.05);观察组人工气道留置时间显著低于对照组(P<0.05)。结论高流量吸氧装置应用于撤机后人工气道留置患者中效果显著,能改善患者的氧合状态,减少SBT患者的人工气道留置时间。
Objective To explore the effect of high flow device on the retention time of artificial airway after removal.Methods We collected 394 patients from July 2017 to December 2017 in ICU as the observation group,who had artificial airway mechanical ventilation,between January 2017 and June 2017 in ICU of our hospital 357 patients with artificial airway were retained as the control group,two groups to evaluate patients with artificial airway lien and MV,weaning patients with indications for 30 minutes to 120 minutes of spontaneous breathing trial(SBT)and air leakage test.Observation group with high flow oxygen therapy device,While the control group used the T tube oxygen.Comparing age,gender,APACHE Ⅱ scores,the blood gas analysis index of 30 minutes after withdrawal:pH,PaO 2,PaCO 2,RR,oxygen index(PaO2/ FiO2),and artificial airway retention time between two groups.Results (1)There was no statistically significant difference in gender,age,APACHE Ⅱ scores and other general information in two groups( P > 0.05 ).(2)The blood gas analysis indexes of 30 minutes after the withdrawal of the observation group:pH,PaO2,PaCO 2,RR,and oxygen index(PaO2/ FiO2)were significantly higher than the control group( P < 0.05 ).(3)The artifical airway retention time of observation group was significantly lower than the control group( P < 0.05 ).Conclusion The high flow oxygen inhalation device is effective in the patients with artificial airway retention after withdrawal,which can improve the oxygenation status of patients and reduce the time of artificial airway retention in SBT patients.
