脐血cTnI及胎心电子监护对窒息新生儿心肌损伤的预测价值 被引量:3

摘要 目的探讨心肌肌钙蛋白I(cTnI)在窒息新生儿脐血及血清中的表达及临床意义。方法选取2015年7月至2018年12月在本院产科分娩的79例足月窒息新生儿,其中窒息合并心肌损伤组19例,窒息不合并心肌损伤组60例,分别检测各组脐血、急性进展期血清中cTnI的表达水平。同时选取100例同期正常分娩的足月健康新生儿作为对照组,均检测脐血cTnI水平,对比各组情况并进行统计学分析。结果无心肌损伤的窒息组中脐血及急性进展期的cTnI水平分别为(0.20±0.12)ng/ml、(0.64±0.15)ng/ml,差异有统计学意义(P<0.05);合并有心肌损伤的窒息组中脐血及急性进展期的cTnI水平分别为(0.51±0.24)ng/ml、(1.18±0.36)ng/ml,差异有统计学意义(P<0.05);对照组脐血中的cTnI表达水平为(0.09±0.08)ng/ml,明显低于窒息组,三组比较差异均有统计学意义(均P<0.05)。窒息组中合并有严重异常胎监图形的共39例,合并心肌损伤组18例,无心肌损伤组21例,两者比较差异有统计学意义(P<0.05)。结论异常的胎监图形联合脐血cTnI的早期检测有助于及早发现窒息新生儿中的心肌损伤患者,有助于早期干预及治疗,降低新生儿不良预后。 Objective To study the expression and clinical significance of myocardial troponin I in cord blood and serum of asphyxia newborn umbilical cord. Methods 79 full-term asphyxia neonates, including 19 cases of suffocation merger myocardial injury and 60 cases of no merge myocardial injury, were selected. The expression of cTnI was detected both in umbilical cord blood and in serum during acute phase progress. 100 fullterm healthy newborns were selected as a control group at the same time;the cTnI umbilical cord blood was detected. The results of the two groups were compared. Results The cTnI levels in umbilical cord blood and acute phase progress of the no merge myocardial injury group were (0.20±0.12) ng/ml and (0.64±0.15) ng/ml, respectively, with a statistical difference (P < 0.05). The cTnI levels in umbilical cord blood and acute phase progress of the suffocation merger myocardial injury group were (0.51±0.24) ng/ml and (1.18±0.36) ng/ml, respectively, with a statistical difference (P < 0.05). The cTnI level expression in umbilical cord blood of the control group (0.09±0.08) ng/ml, and was significantly lower than those of the mild asphyxia group and severe asphyxia group, with statistical differences (P < 0.05). 39 cases had severe abnormal fetal monitor graphics in the asphyxia group, 18 cases in the myocardial injury group, and 21 cases in the no merge myocardial injury group, with statistical differences (P < 0.05). Conclusion Abnormal fetal monitor graphics combined with early detection of cTnI in umbilical cord blood is helpful for early detection of asphyxia newborns with myocardial injury, and is useful for early intervention and treatment, so it can reduce the poor prognosis in neonates.
