摘要
本文介绍临床药师参与1例肺癌患者癌痛联合治疗的过程,临床药师根据患者病情变化,结合文献资料,分析突发血压升高的原因,确定为盐酸羟考酮缓释片与盐酸舍曲林片的相互作用引起5羟色胺综合征的不良反应。临床药师探讨阿片类药物和抗抑郁药物的相互作用,并提出进一步治疗措施。在临床治疗过程中,由于疾病共存,常有联合用药,临床药师应熟悉各种药物的药理学和药动学特征、不良反应及可能存在的药物相互作用,减少或避免不良反应的发生,促进临床安全合理用药。
The article introduced the participation process of clinical pharmacists in the combined treatment of cancer pain in a case of lung cancer. According to the changes of the patient’s conditions and a large number of literatures,clinical pharmacists analyzed the causes for the sudden elevated blood pressure,and determined the interaction between oxycodone and sertraline causing the serotonin syndrome. Clinical pharmacists explored the interaction between opioids and antidepressants,and proposed further treatment regimens. The phenomenon of concomitant drugs is frequent due to the coexistence of diseases in the course of clinical treatment,clinical pharmacists should be familiar with the pharmacological and pharmacokinetic characteristics of various drugs,adverse reactions and possible drug interactions in order to reduce or avoid the occurrence of adverse reactions and promote clinically safe and rational use of drugs.
