摘要
目的探讨实施医联体对建档立卡贫困人口慢性病管理的影响及应用效果。方法采用整群抽样方法对某镇5个贫困村的高血压患者和糖尿病患者建档立卡,其中高血压患者504人,糖尿病患者77人。于2018年始,采用现行医联体模式下进行健康管理,比较实施前后患者病情控制情况。结果实施医联体建档立卡后,4个村中高血压疾病的控制效果得到提高,在3个村中对糖尿病疾病病情控制较好,差异均有统计学意义(P<0. 05);总体来看,方案实施后,该人群高血压和糖尿病两种慢性疾病控制率明显改善,差异有统计学意义(P<0. 05)。结论医联体模式下对贫困人口高血压及糖尿病两种慢性疾病的建档立卡有利于病情控制;医联体建档立卡模式在不同贫困村,对于高血压和糖尿病病情控制情况不同,要因地制宜的制定指导措施。
Objective To explore the influence and application effect of using the medical association to establishing card on the chronic disease management of poor population.Methods Cluster sampling was used to establish archives of hypertension and diabetes patients in 5 poor villages,in which 504 patients were hypertension and 77 were diabetes.The medical association model was used for health management beginning in 2018. Patient control was compared before and after implementation of the model. Results After implementation of the model,control effect of hypertension in four villages and control effect of diabetes in three villages were increased (P < 0. 05).From the overall results,the control effect of hypertension and diabetes was significantly improved after the implementation of the model (P < 0. 05).Conclusion Establish archives of hypertension and diabetes in poor population under the model of the medical association is beneficial to disease control in poor population. However,the targeted model should be formulated according to local conditions since there are differences in various poverty-stricken villages.
