血清B型脑钠肽前体水平与老年2型糖尿病性心肌病变患者左心室功能的关系 被引量:5

The relationship between serum levels of N-terminal pro-brain natriuretic peptide and left ventricular function in elderly patients with type 2 diabetic cardiomyopathy

摘要 目的探讨血清B型脑钠肽前体(NT-proBNP)水平与老年2型糖尿病(T2DM)性心肌病变患者左室功能的关系。方法我院收治的老年T2MD性心肌病变患者57例(心肌病组),同期50例单纯老年T2DM患者(T2DM组),采集患者外周血标本测定NT-proBNP水平,并采用超声心动图测定患者左室功能,包括左室射血分数(LVEF)、左心房收缩末期容积指数(LAVI)、舒张早期二尖瓣血流速度峰值(E)、舒张晚期二尖瓣血流速度峰峰值(A)、舒张早期二尖瓣环运动速度(E’),计算E/A、E/E’值,分析血清NT-proBNP水平与左室功能的关系。结果心肌病组血清NT-proBNP水平及LAVI、E/E’值高于T2DM组,E/A值低于T2DM组(P<0. 05);T2DM心肌病变患者血清NT-proBNP水平与LAVI、E/E’值呈正相关(P<0. 05)。结论老年T2DM性心肌病变患者血清NT-proBNP水平高于单纯T2DM患者,且NT-proBNP水平的变化与左室舒张功能指标LAVI、E/E’值呈正相关,可反映左室舒张功能变化程度。 Objective To explore the relationship between serum levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) and left ventricular function in elderly patients with type 2 diabetes mellitus (T2DM) cardiomyopathy.Methods Fifty-seven elderly patients with T2MD cardiomyopathy in our hospital were selected as cardiomyopathy group.Another 50 patients with simple T2DM in our hospital were selected as T2DM group.The peripheral blood samples were collected to determine the NT-proBNP levels.The left ventricular function,including left ventricular ejection fraction (LVEF),left atrial end-systolic volume index (LAVI),early diastolic mitral peak flow velocity (E),end diastolic mitral peak flow velocity (A) and early diastolic mitral annulus velocity (E’) were measured by echocardiography.The E/A and E/E’ ratios were calculated.The relationship between serum NT-proBNP levels and left ventricular function in elderly patients with T2DM cardiomyopathy was analyzed.Results The serum NT-proBNP levels,LAVI and E/E’ were higher and the E/A value was lower in the cardiomyopathy group than those in the T2DM group (P<0. 05).The serum NT-proBNP levels in the cardiomyopathy group were positively correlated with LAVI and E/E’ values (P< 0. 05). Conclusion The levels of serum NT-proBNP in elderly patients with T2DM cardiomyopathy is higher than that in patients with simple T2DM.The change of NT-proBNP level is positively correlated with left ventricular diastolic function indexes LAVI and E/E’,which can reflect the change degree of left ventricular diastolic function.
