摘要
目的分析乳腺BI-RADS 3~5类钙化性病变的X射线表现特点,并与磁共振成像(MRI)进行对比。方法我院79例乳腺BI-RADS 3~5类钙化性病变患者,均进行乳腺X射线检查和双乳MRI扫描,分析记录BI-RADS 3~5类钙化性病变X射线表现特点和MRI影像学特征,并以术后病理检查为金标准,比较两种方法对BI-RADS 3~5类钙化性病变诊断信效度。结果乳腺X射线图像显示BI-RADS 3类、4a类、4b类、4c类、5类钙化性病变钙化形态分别主要为圆点状、无定形模糊状、粗糙不均匀状、细微多形性、分支状蠕虫样,主要分布类型依次为成簇、成簇、成簇、段样、段样分布。MRI图像显示BIRADS 3类、4a类、4b类、4c类、5类钙化性病变形态分别为边缘光滑肿块、边缘光滑或呈分叶状肿块、边缘不光滑圆形肿块、边缘毛刺状圆形肿块、边缘不光滑或毛刺状圆形肿块,主要内部强化特点依次为环形强化、均匀强化、不均匀强化、不均匀强化、不均匀强化。乳腺X射线对BI-RADS 3~5类钙化性病变总准确度为72. 15%,低于MRI对BI-RADS 3~5类钙化性病变总准确度87. 34%(P<0. 05)。乳腺X射线和MRI对BI-RADS 3~5类钙化性恶性病变诊断灵敏度、特异度、准确度分别为91. 07%、65. 22%、83. 54%和89. 29%、86. 96%、88. 61%。结论乳腺BI-RADS 3~5类钙化性病变的X射线和MRI图像均有相应特点,且MRI对BI-RADS 3~5类钙化性病变诊断准确度和恶性病变诊断特异度更高。
Objective To analyze the X-ray features of breast BI-RADS 3 ~ 5 type calcified lesions and compare them with magnetic resonance imaging( MRI).Methods Seventy-nine patients with breast BI-RADS 3 ~ 5 type calcified lesions in our hospital underwent mammography and double-breast MRI scan.The X-ray features and MRI findings were recorded and analyzed.With postoperative pathology as the standard,the reliability and validity were compared between the two methods for diagnosis of BI-RADS 3 ~ 5 type calcified lesions.Results Mammographic examination showed that the shapes of BI-RADS 3,4a,4b,4c and 5 types of calcified lesions mainly were point-shaped,amorphous-fuzzy,rough-uneven,tiny-pleomorphic and branched worm-like. The main distribution types were clustered,clustered,clustered,segment-like and segment-like,respectively. The MRI images showed that BI-RADS 3,4a,4b,4c and 5 types of calcified lesions were masses with smooth edges,masses with smooth edges or lobulated masses,round masses with rough edges,round masses with burr-like edges and round masses with rough or burr-like edges,respectively.The main features of internal enhancement were ring enhancement,homogenous enhancement,and inhomogenous enhancement.The total accuracy of mammography for BI-RADS 3 ~ 5 type calcified lesions was lower than that of MRI (72. 15% vs.87. 34%)(P<0. 05).The sensitivities,specificities and accuracy rates of mammography in the diagnosis of BI-RADS 3 ~ 5 calcified malignant lesions were 91. 07%,65. 22%,and 83. 54%,respectively and MRI were 89. 29%,86. 96%,88. 61%,respectively. Conclusion The X-ray and MRI images of breast BI-RADS 3 ~ 5 type calcified lesions have corresponding characteristics,and the diagnostic accuracy of MRI is higher.
