高压氧对体外冲击波碎石术后肾脏缺血再灌注损伤的保护作用

摘要 目的 探讨高压氧对体外冲击波碎石术(ESWL)后肾脏缺血再灌注损伤的保护作用。方法 60例经ESWL治疗后出现肾脏缺血再灌注损伤的患者随机分为试验组和对照组,每组30例。试验组于ESWL后第2天起予以高压氧疗60min/次,治疗压力0.2MPa,1次/d,共5次;对照组于ESWL后给予常规处理。观察治疗前后两组患者肾主动脉阻力指数(MRARI)、肾叶间动脉阻力指数(IRARI)、血尿素氮(BUN)、肌酐(Cr)、丙二醛(MDA)、超氧化物歧化酶(SOD)、内皮素-1(ET-1)、尿微量白蛋白(mAlb)等指标变化,分析高压氧对ESWL后肾脏缺血再灌注损伤的保护作用。结果 试验组和对照组患者碎石前和碎石后第1天、第6天、第10天的MDA、SOD、ET-1、mAlb、MRARI、IRARI比较差异均有统计学意义(P<0.05),两组患者BUN、Cr在碎石前后比较差异无统计学意义(P>0.05)。两组碎石后第1天、第6天、第10天与碎石前MDA、SOD、ET-1、mAlb、MRARI、IRARI指标的差值比较,差异均有统计学意义(P<0.05)。结论 高压氧对体外冲击波碎石后肾脏缺血再灌注损伤具有保护作用。 Objective To investigate the protective effect of hyperbaric oxygen on renal ischemia -reperfusion injury afteRextracorporeal shock wave lithotripsy(ESWL). Methods Sixty patients with renal ischemia-reperfusion injury afteRESWL treatment were randomly divided into experimental group and control group, with 30 cases in each group. The experimental group initiated hyperbaric oxygen therapy foR60 minutes peRtime on the 2nd day afteRESWL, with a treatment pressure of 0.2 MPa, once daily foR5 times. The control group was given conventional treatment afteRESWL. The changes in main renal artery resistance index (MRA-RI), interlobaRrenal artery resistance index (IRA-RI), blood urea nitrogen (BUN), creatinine (Cr), malondialdehyde (MDA), superoxide dismutase (SOD), endothelin-1 (ET-1), and urinary microalbumin (mAlb) before and afteRtreatment were observed in the two groups, the protective effect of hyperbaric oxygen on renal ischemia-reperfusion injury afteRESWL was analyzed. Results Before lithotripsy and on the1 st, 6th and1 0th day afteRlithotripsy, there were statistically significant differences in MDA, SOD, ET-1, mAlb, MRA-RI and IRA-RI between the experimental group and the control group ( P <0.05), no statistically significant difference was found in BUN oRCRbetween the two groups before and afteRlithotripsy ( P >0.05). The differences between indicators of MDA, SOD, ET-1, mAlb, MRA-RI, and IRA-RI before lithotripsy and those on the1 st, 6th or1 0th day afteRlithotripsy showed statistically significant difference between the two groups( P <0.05 ). Conclusion Hyperbaric oxygen has a protective effect on renal ischemia-reperfusion injury afteRESWL.
