摘要
目的探讨降钙素原( PCT)、C -反应蛋白( CRP)在指导抗生素治疗细菌感染中的意义。方法选取 2018 年 1 月- 7 月细菌培养阳性患者 89 例为研究对象,根据细菌培养结果分为革兰阳性菌感染组和革兰阴性菌感染组,抽取同期健康体检者 100 例为对照组,全部患者于治疗前、使用抗菌药物 1 d、3 d、5 d、7 d 测定血清 PCT 与 CRP 水平并比较分析,同时分析 PCT 与 CRP 诊断细菌感染的效能。结果细菌感染组 PCT 与 CRP 水平均高于对照组( P < 0. 05)。革兰阴性菌感染者血清 PCT 水平显著高于革兰阳性菌感染者,差异均有统计学意义( P < 0. 05),而 CRP 水平在两者之间差异无统计学意义( P > 0. 05)。经有效治疗 7 d 后,细菌感染组 PCT 与 CRP 水平均明显下降,差异均有统计学意义( P < 0. 05)。PCT 诊断细菌感染的敏感度为 86. 5%,特异度为 90. 0%,阳性预测值为 88. 5%,阴性预测值为 88. 2%, CRP 分别为 73. 0%、79. 0%、75. 6%、76. 7%。结论联合检测 PCT 与 CRP 水平在细菌感染的早期诊断、疗效评估中有一定价值,且 PCT 对鉴别革兰阳性菌及革兰阴性菌感染有较高的价值。
Objective To explore the application value of procalcitonin ( PCT) and C-reactive protein ( CRP) in guiding antibiotics to treat bacterial infection. Methods A total of 89 patients with positive bacterial culture from January 2018 to July were selected as the research subjects,according to the results of bacterial culture,it was divided into Gram-positive ( G +) infection group and Gram-negative ( G -) infection group,100 healthy people in the same period were randomly selected as the normal control group,all patients were tested for serum PCT and CRP levels and compared with antibiotics at d 1,d 3,d 5 and d 7 before admission. At the same time,the efficacy of PCT and CRP in diagnosing bacterial infection was analyzed. Results The PCT and CRP levels in the bacterial infection group were higher than those in the control group ( P < 0. 05). The PCT level of G-bacteria infection was significantly higher than that of G + bacteria infection ( P < 0. 05),but there was no statistical significance on the difference in CRP level between the two groups ( P > 0. 05). After 7 days of effective antimicrobial treatment,the levels of PCT and CRP in bacterial infection group were significantly lower than those before treatment ( P < 0. 05). The sensitivity of PCT for diagnosis of bacterial infection was 86. 5%,specificity was 90. 0%,positive predictive value was 88. 5%,negative predictive value was 88. 2%,and CRP was 73. 0%,79. 0%,75. 6%,and 76. 7%,respectively. Conclusion Combined detection of PCT and CRP levels has certain value in the early diagnosis and evaluation of bacterial infections,and PCT has a high clinical value for the identification of G + and G-bacteria infections.
