摘要 目的:北京同仁医院眼科手术中心一日病房每日的手术量巨大,现有流程患者手术日需多次排队等候,导致患者总等候时长过久且等候区就医秩序混乱.方法:利用信息化工具设计一套分时预约、排队叫号的新就医流程以达到缩短患者手术日总等候时长,降低人流密度,改善就医环境的目的.系统设计采用临床用户调研、专家访谈方式采集一手数据,基于数据分析并建立需求模型,以瀑布模型作为开发设计工具.利用、改造现有HIS与叫号集成硬件资源以降低研发、实施成本.结果:流程改造实施后,通过等候患者时间对比与临床用户访谈手段评估系统运行效果并改进不足,不断完善流程以期最终实现改善医患双方均体验不佳的现状.结论:日间手术分时预约叫号系统可以有效缓解患者等候时间,改善就医环境,提升医患双方体验.结合日间手术的工作特点改进管理流程,可以有效促进日间手术发展. Objective: There is a great quantity of ambulatory surgeries in Beijing Tongren hospital ophthalmic day-care unit. Patients with existing processes has to wait multiple times in line, as a result the patients' total waiting time was too long, the waiting area was so congest and the medical order was disorganized. Methods: To design a time-sharing appointment process and a queue system with information technology in order to shorten the patient's total waiting time on the operation day, reduce the flow of people to improve the medical environment. Primary data collected from clinical user research and expert interviews for the system design. Based on the analysis of data collected to establish the requirement modeling, use the waterfall model as a development tool. Utilize the existing HIS and the hardware resources to reduce the implementation costs. Results: By comparisons the patient waiting time and clinical users interviews after the implementation to evaluate system performance and improve shortcomings, the process is continuously improved so as to finally improve the poor status of both doctors and patients sides. Conclusion: Ambulatory surgery time-sharing appointment and queue system can effectively alleviate patient waiting time to improve the medical order and also improve the experience of both doctors and patients. Combining the working characteristics of ambulatory surgery to improve the management process can effectively promote the evolution of ambulatory surgery.
