摘要
目的:系统评价复方苦参注射液联合同步放化疗治疗中晚期宫颈癌的疗效及安全性。方法:计算机检索Pubmed、Embase、Cochrane图书馆、中国生物医学文献数据库、中文科技期刊全文数据库、万方数字化期刊全文库、中国期刊全文数据库,收集2017年11月以前关于复方苦参注射液联合同步放化疗治疗宫颈癌的文献,按照纳入排除标准筛选。Meta分析使用RevMan 5.3软件进行。结果:纳入9项RCT研究,共804例患者。Meta分析结果显示:与单纯放化疗相比,复方苦参注射液联合同步放化疗可以提高近期临床有效率[OR=2.21,95%CI(1.58,3.09),P<0.000,01),提高生存质量[OR=3.20,95%CI(2.15,4.77),P<0.000,01)、降低胃肠道反应发生率[OR=0.5,95%CI(0.36,0.71),P=0.000,1),降低白细胞下降发生率[OR=0.34,95%CI(0.22,0.53),P<0.000,01),增强免疫功能(P<0.05)。结论:复方苦参注射液联合同步放化疗治疗中晚期宫颈癌有较好疗效并能降低毒副反应,值得临床推广应用。由于纳入文献的方法学质量不高,以上结论有待高质量的RCT进一步证实。
Objective: To evaluate the efficacy and safety of Compound Kushen injectionin combination with concurrent chemoradiotherapy in the treatment of advanced cervical cancer. Methods: We set include and exclude criterion and searching strategy, then databases of PubMed, EMBase, Cochrane, CBM, VIP, Wanfang, CNKI we researched, and randomized control trials of compound kushen injection combined with chemotherapy in the treatment of cervical cancer before November 2017 were gathered. Meta-analysis was performed using the Cochrane RevMan 5.3 software. Results: Nine randomized controlled trials we included, accounting for 804 cases.Meta-analysis showed that the combination of Compound Kushen injectionin combination with concurrent chemoradiotherapy can improve the short-term clinical efficacy [OR=2.21, 95%CI(1.58,3.09), P<0.000,01], improve the quality of life [OR=3.20, 95%CI(2.15,4.77), P<0.000,01), decrease the incidence of gastrointestinal reactions[OR =0.5, 95% CI(0.36,0.71), P=0.000,1], reduce the incidence of leucopenia [OR=0.34, 95% CI(0.22,0.53), P<0.000,01] and enhance immune function(P<0.05). Conclusion: The combination of compound Kushen injection combined with concurrent chemoradiotherapy for advanced cervical cancer can improve the clinical efficiency and reduce toxic and side effects. Due to the poor methodological quality in these study, these conclusion need stobe further confirmed by high quality RCTS.
