摘要
目的 探讨听神经瘤患者术前应用弥散张量成像(diffusion tensor imaging,DTI)定位面神经的可行性。方法 对6例听神经瘤患者术前进行DTI扫描定位面神经,将结果与术中所见面神经位置(分为肿瘤的上级、下级、前方和后方)比较,验证DTI的准确性。结果 6例患者中5例术前DTI面神经的重建结果与术中所见一致,DTI的准确率为 83.3%(5/6),且DTI及术中面神经定位均显示面神经在肿瘤前方;1例患者术前DTI显示面神经在肿瘤前方,而术中见肿瘤压迫致面神经移位变形至肿瘤上极。结论 DTI对听神经瘤患者术前面神经定位有较高的准确率及良好的应用前景,有助于确定最佳手术路径、缩短寻找面神经的时间,降低面神经损伤概率。
Objective To study the accuracy of locating facial nerve in patients with acoustic neuroma before operation by diffusion tensor imaging(DTI).Methods Six patients with acoustic neuroma were preoperatively subjected to DTI scanning to locate the facial nerve. The accuracy of the examination was verified by comparison with the results obtained during the operation. The difficulty of recording and analyzing the DTI was applied to the further development of patients with acoustic neuroma.Results All the 6 patients underwent reconstruction of the facial nerve before operation. The reconstruction results of 5 patients were consistent with those seen during the operation. The reconstruction results and the location of facial nerve surgery were in the front of the tumor. DTI has high accuracy. However, long inspection time and high professional knowledge were the difficulties of improving before clinical application.Conclusion DTI has high accuracy and good application prospects for the operation of anterior nerve localization in patients with acoustic neuroma. It can help surgeons to determine the optimal surgical path, shorten the time to find facial nerve, and reduce the probability of facial nerve injury along with the tumor recurrence rate.
