摘要
目的:观察基于养阴清肺法组方的抗痨合剂对耐多药肺结核患者Th17/Treg细胞的影响。方法:采用前瞻性研究方法,选取丽水地区2012年1月-2017年12月耐多药肺结核患者87例,随机分为对照组与观察组。对照组予以常规西药抗结核治疗方案,观察组在对照组的基础上强化期辅以抗痨合剂治疗。采用流式细胞术、ELISA法检测两组强化期治疗前后Th17、Treg细胞的含量及相关细胞因子水平。结果:两组患者完成6个月强化期治疗的耐多药肺结核患者共计71例,其中对照组33例,观察组38例。研究结果显示,观察组患者外周血Th17细胞含量、Th17/Treg值与对照组比较均升高,差异有统计学意义(P <0. 05),观察组耐多药肺结核患者Treg细胞的含量低于对照组,差异有统计学意义(P <0. 05);观察组耐多药肺结核患者外周血中IL-17的水平与对照组比较显著升高,差异有统计学意义(P <0. 05),而TGF-β水平与对照组比较明显降低,差异有统计学意义(P <0. 05)。结论:抗痨合剂可能是通过上调耐多药肺结核患者体内Th17水平,下调Treg水平,改善Th17/Treg的平衡状态,从而发挥抗结核治疗作用。
Objective:To study the effect of Kanglao Mixture on Th17/Treg cells in patients with multidrug-resistant tuberculosis( MDR-TB).Methods:A prospective study module was used to analyze the therapeutic outcome of MDR-TB for 87 patients between January 2012 to December 2017 in Lishui area.The 87 patients with MDR-TB were randomly divided into two groups:control group and observation group.The control group was treated with formal standardized anti-tuberculous with Western Medicine.The observation group was treated with Kanglao Mixture and formal standardized anti-tuberculous with Western Medicine.After the period of treatment,all the patients were observed the clinical effects of drugs.Venous blood in both groups were taken to detect Treg and Th17 cells contents and related cytokines levels,and the detection outcomes were analyzed.Results:A total of 71 patients with MDR-TB completed 6 months intensive treatment in the two groups,including33 cases in the control group and 38 cases in the observation group.The results showed that Th17 cell counts and Th17/Treg ratio in the observation group were significantly higher than that in the control group in peripheral blood of patients( P < 0.05),but Treg cell counts in the observation group were lower than that in the control group( P < 0.05).The level of IL-17 in peripheral blood of the observation group was significantly higher than that of control group( P < 0.05),and the level of TGF-β was significantly lower than that of control group( P< 0.05).Conclusion:Kanglao Mixture may improve balance of Th17/Treg by upregulating content of Th17 cells and down-regulating content of Treg cells in patients with MDR-TB,so as to play the role of anti-tuberculosis therapy.
