自我管理教育在动静脉内瘘成形术患者中的应用 被引量:2

摘要 目的探讨自我管理教育在动静脉内瘘成形术患者中的应用效果。方法将60例动静脉内瘘成形术患者按随机数字表法分为对照组和试验组,每组30例。对照组给予常规健康教育,试验组在对照组的常规健康教育的基础上给予自我管理教育。观察2组出院时,出院后1、3个月自我管理能力(包括保持良好的生活习惯、自我处置药物行为、自我监控症状行为、心理自我调整行为、出现新问题及时处理和定期复诊)评分和出院后3个月生活质量评分(包括功能领域、症状领域评分)及内瘘并发症(内瘘功能不良、内瘘血栓、内瘘狭窄、内瘘血流量不足)发生率。结果试验组出院后1、3个月保持良好的生活习惯、自我处置药物行为、自我监控症状行为、心理自我调整行为、出现新问题及时处理和定期复诊评分均高于对照组,差异有统计学意义(P<0.05)。试验组出院后3个月情感功能、总体健康状况评分均高于对照组,疼痛、睡眠障碍评分和内瘘功能不良、内瘘狭窄、内瘘血栓、内瘘血流量不足发生率均低于对照组,差异有统计学意义(P<0.05)。结论自我管理教育能显著提高动静脉内瘘成形术血液透析患者的自我管理能力,减少内瘘并发症的发生,提高患者的生活质量。 Objective To explore the efficacy of self-management education in patients undergoing arteriovenous fistula plasty. Methods Sixty patients undergoing arteriovenous fistula plasty were randomly given routine health education alone(control group, n =30) or in combination with self-management education(experimental group, n =30).The scores of self-management abilities(maintaining good living habits,self-disposal drug behavior,self-monitoring symptom behavior,self-monitoring fistula function behavior,psychological adjustment behavior,timely treatment of new problems and regular follow-up visit) were observed at discharge and 1 and 3 months after discharge.The quality of life scores(functional and symptomatic areas) and the incidences of complications(internal fistula dysfunction,thrombosis,stenosis and blood flow velocity) were measured 3 months after discharge. Results The scores of maintaining good living habits,self-disposal drug behavior,self-monitoring symptom behavior,self-monitoring fistula function behavior,psychological adjustment behavior,timely treatment of new problems and regular follow-up visit in experimental group were higher than those in control group 1 and 3 months after discharge( P < 0.05 ).Furthermore,compared with control group,the scores of emotional state and total health status increased and the scores of pain and insomnia and incidences of dysfunction,stenosis,thrombosis and insufficient blood flow of internal fistula decreased in experimental group 3 months after discharge( P <0.05). Conclusion Self-management education can enhance the self-management ability,prevent the occurrence of complications and improve the quality of life in patients undergoing long-term arteriovenous fistula plasty.
