摘要
目的:观察活血颗粒对急性脑梗死血瘀证患者血清炎症因子、血瘀积分、神经功能的影响。方法:将急性脑梗死血瘀证患者128例,随机分为2组。对照组给予西医基础治疗,治疗组在西医基础治疗上加服活血颗粒,每次1袋,开水冲服,每日3次。分别于入院后治疗前、治疗第7天、治疗第14天检测患者血清白介素-1β(IL-1β)、白介素-6(IL-6)、白介素-10(IL-10)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)含量以及血瘀积分与美国国立卫生研究院卒中量表(NIHSS)评分。结果:两组治疗后第7天、第14天血瘀积分与治疗前比较显著降低(P<0.05或P<0.01),且两组治疗后第14天比较均有显著性差异(P<0.05)。两组CRP、TNF-α水平在治疗后第7天、治疗后第14天均较治疗前显著降低(P<0.01),且两组比较差异具有统计学意义(P<0.01);两组治疗后第7天、治疗后第14天IL-6、IL-1β水平均较治疗前显著降低(P<0.01),两组治疗后IL-1β水平比较差异均具有统计学意义(P<0.01),两组IL-10水平在治疗后第7天时较治疗前升高(P<0.01),治疗后第14天较治疗后第7天降低(P<0.01),两组间比较均差异显著(P<0.01)。结论:活血颗粒具有改善急性脑梗死患者神经功能及血瘀程度的作用,可能与其上调IL-10表达,下调IL-1β、TNF-α表达等机制有关。
Objective:To observe the effects of Blood-Activating granules on serum inflammatory factors, blood stasis score, and neurological function(NIHSS score)in patients with blood stasis due to acute cerebral infarction. Methods: 128 ACI blood stasis patients were randomly divided into 2 groups. The control group was given the basic treatment with western medicine. In the treatment group, blood-Activating granules were added besides the basic treat. ment of western medicine. For each time one bag of granule was taken with boiled water, 3 times a day. The IL-1β, IL-6, IL-10, CRP, TNF-α, blood stasis score as well as NIHSS score in serum content were tested before treatment, after 7 days and 14 days. Result: The blood stasis scores decreased significantly from the two groups after 7 days and after 14 days(P<0.05 or P<0.01), and there was significant difference between the two groups after 14 days(P<0.05). The CRP, TNF-α decreased significantly after 7 days and 14 days(P<0.01),and there was statistic difference between two groups(P<0.01). The IL-6、IL-1β decreased significantly after 7 days and 14 days(P<0.01),and there was statistic difference in comparing of IL-1β. The IL-10 increased after 7 days of treatment(P<0.01),and it decreased after 14 days comparing the previous value after 7 days, there was significant difference in the 2 groups after 14 days.Conclusion: The nerve function and blood stasis level of acute cerebral infarction reduce (ACI) patients can be im? proved by Blood-Activating granules, the therapeutic achievement may be related to up-regulated IL-10 expression, down-regulatedIL-1β,TNF-αexpression etc.
