摘要
目的探讨诱导痰细胞学分类与中性粒细胞-淋巴细胞比值(NLR)、血小板-淋巴细胞比值(PLR)在评估急性发作期哮喘患者病情严重程度中的作用。方法回顾性分析山西医科大学第一医院呼吸科2017年1月至2018年1月住院的85例急性发作期哮喘患者的临床资料,记录其年龄、性别、体质量指数(BMI)、病程和吸烟率等一般资料,收集诱导痰细胞学分类、血细胞分析并计算其中NLR、PLR、肺功能、动脉血气等临床资料。根据病情严重程度将其分为轻、中、重度3组。根据诱导痰细胞学分类,分为嗜酸粒细胞(EOS)型、中性粒细胞(NEU)型、混合细胞(MIX)型、寡细胞(PAU)型4组,比较NLR、PLR在病情严重程度及不同表型之间的差异。结果不同严重程度哮喘患者的表型构成比差异有统计学意义(χ^2=12.42,P<0.05)。重度哮喘患者NLR、PLR显著大于轻、中度,差异有统计学意义(χ^2=16.70、11.67,P值均<0.01)。痰EOS%、痰NEU%及血细胞分析中EOS%、NEU%在4组炎症表型间比较,差异有统计学意义(χ^2=62.76、68.17、6.96、28.50,P值均<0.01)。NLR、PLR在NEU型哮喘患者中显著升高,在4组间差异有统计学意义(χ^2=11.34、3.01,P值均<0.05)。结论NLR、PLR值越高,痰NEU%越高,NEU型哮喘可能性越大,病情越严重,肺功能越差。
Objective To investigate the role of induced sputum and neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) in assessing the severity of asthma patients. Methods The clinical data of 85 patients with acute asthma who were hospitalized from January 2017 to January 2018 in the First Hospital of Shanxi Medical University were retrospectively reviewed.General information such as age, sex, body mass index (BMI), duration of disease and smoking rate were collected.Induced sputum, blood cell analysis and calculation of NLR and PLR, pulmonary function, arterial blood gas and other clinical data.According to the severity of the disease, it was divided into three groups: light, medium and severe.According to the induced sputum, it was divided into four groups: eosinophil (EOS) type, neutrophil (NEU) type, mixed (MIX) type, and paucigranulocytic (PAU) type, and NLR and PLR were compared.The difference between the severity of the disease and the different phenotypes. Results The phenotypic composition ratio of asthma patients with different severity was statistically significant (χ^2=12.42, P<0.05). NLR and PLR were significantly higher in patients with severe asthma than in mild to moderate (χ^2=16.70, 11.67, both P<0.01). sputum EOS%, sputum NEU% and blood EOS%, blood NEU% were compared between the four groups of inflammatory phenotypes, the difference was statistically significant (χ^2=62.76, 68.17, 6.96, 28.50, all P<0.01). NLR and PLR were significantly elevated in patients with NEU type asthma, and the difference was statistically significant among the four groups (χ^2=11.34, 3.01, both P<0.05). Conclusions The higher the NLR and PLR values, the higher the sputum NEU%, the greater the possibility of NEU type asthma, the more serious the disease, and the worse the pulmonary function.
