摘要
目的探讨基于手机APP的个案管理在乳腺癌术后患者功能锻炼中的应用效果。方法采用便利抽样法,选取2017年1—12月于郑州市某三级甲等医院乳腺外科行腋窝淋巴结清扫术的乳腺癌患者120例为研究对象,采用时间分组的方法,将2017年1—6月住院患者设为对照组,2017年7—12月患者设为干预组,每组各60例。对照组患者术后常规给予患肢功能锻炼指导,干预组患者联合使用手机APP进行个案管理,均随访至术后6个月。比较两组患者水肿发生率、上臂和肩关节功能、生活质量的差异。结果干预组术后1个月、6个月淋巴水肿发生率低于对照组,差异有统计学意义(P<0.05);术后6个月,干预组患者的关节功能、生活质量除情感状况外均优于对照组,差异有统计学意义(P<0.05)。结论对乳腺癌术后患者实施基于手机APP的个案管理,能有效促进患者术后患侧肢体的功能康复,减少术后淋巴水肿的发生,提高其生活质量。
Objective To explore the effect of the mobile-app-based case management on functional rehabilitation among breast cancer patients after operation.Methods A total of 120 breast cancer patients who had undergone axillary lymph node dissection in a ClassⅢGrade A hospital in Zhengzhou city from January to December in 2017 were selected by convenient sampling.According to their admission time,60 in-patients who were admitted between January and June in 2017 were assigned into the control group,and the other 60 in-patients who were admitted between July and December in 2017 were put into the observation group.In the control group,the patients were given by regular functional rehabilitation guidance for the affected limb while in the observation group,the patients were managed by mobile-app-based system besides the regular guidance.For both groups,the follow-up visit lasted for 6 months after surgery.The patients'incidence of edema,upper limbs and shoulder joints'function as well as their quality of life were compared and studied.Results The incidence of lymphedema at one month and six months after surgery in the observation group were lower than the control group with statistical significance(P<0.05).Six months after surgery,the patients in the observation group were better than the control group in terms of the joint function,quality of life except the emotional condition with statistically significant difference(P<0.05).Conclusions The implementation of the mobile-app-based case management for postoperative breast cancer patients can effectively promote the postoperative functional rehabilitation of affected limbs,reduce the occurrence of postoperative lymphedema,and improve their quality of life.
