摘要
视网膜静脉阻塞(RVO)是一种常见的眼底疾病,目前尚无有效治疗方法,而黄斑水肿(ME)是其主要并发症,主要损害中心视力,往往造成患者严重的视力障碍。中医药治疗RVO继发ME被众多临床实践证实是安全有效的,具有低毒、并发症少、不良反应轻、方便、价廉等优点。青蒿为菊科植物黄花蒿的干燥地上部分,而青蒿素是从中提取分离出的一种倍半萜内酯类抗疟药物。近年来研究发现青蒿类药物在感染性疾病、自身免疫性疾病和肿瘤等非疟疾疾病中亦具有显著疗效。青蒿善于滋阴补虚、清热祛湿,而RVO继发ME的主要病机是正虚邪留,正虚者阴虚为主,邪为水湿,与中医治疗RVO继发的ME“扶正祛邪法”一致。本文从中医基础理论出发,从八纲辨证、脏腑辨证、卫气营血三个方面论述青蒿类药物通过“扶正祛邪法”治疗RVO继发ME治疗的理论依据,旨在为临床应用及科研开发提供新的思路。
Retinal vein occlusion(RVO)is a common fundus disease,with no effective treating methods nowadays.Macular edema(ME)is one of its main complications,which may cause severe visual impairment.Traditional Chinese Medicine(TCM)has been proved by many clinical practices to be effective,safe,and convenient with few complications,low cost and low toxicity in treatment of RVO.Artemisiae Annuae Herba is originally derived from the sweet wormwood plant(Artemisia annua).Artemisinin and its derivatives(Artemisinins)are sesquiterpene lactones extracted from this traditional Chinese medicine.Recently discovered properties of artemisinins suggests that they might be used to treat infectious diseases,autoimmune disorders and tumor beyond malaria.Artemisiae Annuae Herba is good at nourishing yin and tonifying deficiency,clearing heat and dispelling dampness.The pathogenesis of RVO is considered to be the healthy qi deficiency and evil retention,typically the yin deficiency being dominant and dampness being the evil,which is consistent with the TCM theory of reinforcing the healthy qi and eliminating pathogen factors in treating ME secondary to RVO.In this paper,we discuss the basis TCM theory of Artemisiae Annuae Herba in treating ME secondary to RVO through reinforcing the healthy qi and eliminating pathogen factors from perspectives of eight principle pattern identification,visceral pattern identification and defense,qi,nutrient and blood pattern identification in order to provide new ideas for scientific researches and clinical studies.
