摘要
目的探讨针刺麻醉诱导对老年患者术后认知功能及炎性因子的影响。方法84例老年骨科手术患者,随机分为研究组、对照组各42例。对照组单纯全麻,研究组在全麻前实施针药诱导。比较2组心率、平均动脉压、简明精神状态量表(MMSE)及血清中炎性因子变化。结果对照组诱导后心率、平均动脉压均低于本组诱导前(P<0.05),且对照组诱导后心率、平均动脉压均低于研究组诱导后(P<0.05)。研究组术后认知功能障碍发生率明显低于对照组(P<0.05)。对照组术后MMSE评分明显低于术前(P<0.05)。术后24h时2组IL-6、TNF-α、IL-10均高于本组诱导前,且对照组均高于研究组(P<0.05)。结论针药麻醉诱导有效改善了老年患者术后认知障碍,抑制炎性因子,疗效较好。
Objective To investigate the effects of acupuncture anesthesia induction on postoperative cognitive function and inflammatory factors in elderly patients.Methods Eighty-four elderly patients underwent orthopaedic surgery were randomly divided into study group and control group, with with 42 cases in each group.The control group was treated with general anesthesia only, and the study group received acupuncture induction before general anesthesia.Heart rate, mean arterial pressure, MMSE and serum inflammatory factors were compared between the two groups.Results Heart rate and mean arterial pressure in the control group were lower than those in the control group before induction (P<0.05), and heart rate and mean arterial pressure in the control group were lower than those in the study group after induction (P<0.05).The incidence of postoperative cognitive dysfunction in the study group was significantly lower than that in the control group (P<0.05).The postoperative MMSE score of the control group was significantly lower than that of the preoperative group (P<0.05).At 24h after surgery, il-6, TNF-, and il-10 in both groups were higher than before induction, and those in the control group were higher than those in the study group (P<0.05).Conclusion Acupuncture and drug anesthesia induction can effectively improve postoperative cognitive impairment in elderly patients, inhibit inflammatory factors, and has good effect.
